Hazardous Chemicals and Gases Monitoring - Wichita VAMC THIS IS A SOURCES SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR PROPOSAL. The VHA Network 15 Contracting Office (NCO 15), 3450 South 4th Street Trafficway, Leavenworth, KS 66048 is performing market research to determine if there is a sufficient number of qualified (1) Service Disabled Veteran Owned Small Business; (2) Veteran Owned Small Business; or (3) Small business firms to set aside a single award contract. The Government will use responses to this notice to make to an appropriate acquisition decision. The requirement is for environmental monitoring for hazardous chemicals and gases in five (5) locations at the Department of Veterans Affairs Medical and Regional office Center, 5500 E. Kellogg, Wichita, KS. Services shall be performed in accordance with the specifications provided below for the Operating Room (OR), Post Anesthesia Care Unit (PACU), Sterile Processing Section (SPS), Endoscopic Processing Room and Laboratory. 1.General Description: a.OR and PACU personnel will be sampled quarterly for Nitrous Oxide, Isoflurane and Sevoflurane. Samples will be taken from four (4) ORs and one (1) PACU and will be conducted under actual conditions of use. One hundred percent (100%) of both the OR and PACU personnel will be sampled over the course of one (1) year. Scheduling will be determined based on personnel scheduling and staffing. b.Air movement (positive and negative pressure) measurements will be taken twice a year in four (4) ORs, one (1) PACU and one (1) SPS Clean Room. c.Conduct Formaldehyde sampling of the PACU Specimen Table semi-annually. Contractor will conduct sampling of the Time Weighted Average/Short Term Exposure Limit (TWA/STEL) personal during their routine activities of pouring and filling. Samples will be taken from the personnel and the room for the Endoscopic Processing Room and the Laboratory. d.Conduct Glutaraldehyde sampling of SPD and Endo semi-annually. Samples will be taken from the personnel and the room for the Endoscopic Processing Room and the Laboratory. The anticipated period of performance will be one basic year plus four (4) one-year pre-priced options for renewal. All interested firms who can meet the requirements stated above should respond, in writing, with information describing their interest, capabilities, geographic location and indicate which small business category for which they qualify to the Contract Specialist, no later than 3:00 p.m. CDT, February 19, 2014. Solicitation Number VA255-14-R-0555 Hazardous Chemicals and Gases Monitoring should be referenced on all correspondence regarding this announcement. The NAICS Code 541380 is applicable to this acquisition with a Business Size Standard of $14 million. Interested firms are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the Central Contractor Registration (CCR) database prior to award of a contract.
