Description

W-9 Form MTC Vendor Self-Certification Form Davis Bacon Wage Schedule Construction Bid Package This is a subcontracting opportunity with Management & Training Corporation (MTC), operator of the Tongue Point Job Corps Center (TPJCC) located at 37573 Old Highway 30, Astoria, OR 97103. MTC is requesting bids for our Sewer Lift Temporary Bypass System (Def.#98243). Services to be provided are outlined in the attached Construction Bid Package (CBP). The Davis Bacon Wage scheduled is provided for your convenience. A pre-bid meeting is scheduled for Wednesday, February 19, 2014 (10:00am (PST) at the TPJCC location •· All bids (to include W-9, Vendor Self-Certification form and bid sheet) must be returned by mail and received no later than Wednesday, March 5, 2013 3:00pm (PST) to Management & Training Corporation, Attn: Sandra Clore, Purchasing Manager, 500 North Marketplace Drive, Centerville, UT 84014. MTC reserves the right to reject any or all bids or waive any formality connected with the bids. MTC will be the sole judge of which bid is the best value and its determination and finding on such decision will be final.
