- SECTION D - CONTRACT DOCUMENTS, EXHIBITS, OR ATTACHMENTS SALIENT SPECIFICATIONS Zeiss Cirris HD-OCT (Model 5000) Spectral domain optical coherence tomography system with line scanning ophthalmoscopy for imaging studies of the retina and optic nerve. Must have DICOM 2.0 conformance and support Integrating the Healthcare Enterprise (IHE) integration profiles. OCT imaging axial resolution in tissue should be capable of less than 7 um and transverse resolution in tissue should be capable of less than 20 um. System should include software to enable viewing of the instruments images from additional workstation other than the equipment's screen. Minimum internal computer memory should be 4 GB. For glaucoma suspects, the system should include software with a normative data base for retinal nerve fiber layer thickness and optic nerve head to which the patient can be evaluated against. For glaucoma patients, the system should include software to facilitate assessment of statistically significant change in retinal nerve fiber layer thickness and optic nerve head changes. For retina patients, the system should include software with a normative data base for macular retinal thickness to which the patient can be evaluated against. The equipment should also include an instrument table with powered height control and excellent ergonomics to facilitate positioning of patients in wheelchairs. Trade in units - (3 each) Cirrus 400 S#4000-3257, 4000-2098, and 4000-7890. STATEMENT OF WORK (SOW) Overview: The winning vendor has 90 days from date of award to perform all tasks listed below. Specific Tasks of Vendor Removing old units Uninstalling and removing from building. Installation of new units Delivery of the units will be to the Vancouver Warehouse. Once inspected and accepted the VA will deliver to their respective locations listed below. At which point, the installation of hardware must be closely coordinated with the Eye Clinic in order to avoid service disruption to the patients. Optimal installation time is outside of normal clinic hours Mon-Fri 0800-1700 Installation is at three locations in the Portland Metro Area: 1.Portland VA Medical Center BLDG, RM 3710 SW US Veterans Hospital Rd Portland, OR 97239 2.Hillsboro Community Based Outpatient Clinic (CBOC) RM? 1925 NW Amberglen Pkway, 3rd floor Hillsboro, OR 97006 3.Vancouver Campus BLDG, RM? 1601 E Fourth Plain Blvd Vancouver, WA 98661 360.695.4061 Confirming functionality - following successful installation, all machines must perform all aspects of software functions and communicate with each other including one unit in place in Hillsboro, one at Vancouver, and another in Portland. There are a total of 5 units that must communicate with one another. A confirmation that each machine functions accordingly will also include verification that all images are accessible from all machines. The vendor will provide a document that functionality has been confirmed. Said document must be signed by Joshua Bunch, Biomedical Equipment Support Specialist for validation. Training Group Training via media presentation will need to be conducted at the Eye Service Staff meeting, what is the location?, which is held from 1400 - 1630 the 2nd Friday of every month for up to 70 personnel. Either before or after the Eye Service Staff meeting, the vendor will provide hands on training for six staff members. The hands on training and media presentation will be approximately 3 hours or training time. Training must take place at the first Eye Service Staff meeting following validation functionality.
