Please note that this is NOT a request for 'Quotes' or 'Proposals', the Department of Veterans Affairs is seeking information for Market Research purposes only. The Department of Veterans Affairs, Phoenix VA Health Care System (PVAHCS) is currently conducting a market survey for qualified firms capable of providing a multi-year (one (1) year base and four (4) option years, not to exceed a duration of sixty (60) months) contract for " Home Infusion Therapy Service ", in accordance with, but not limited to the following: 1.Under Authority Public Law 106-117 and 38 U.S.C. 1720 Home Infusion Therapy Service will be furnished to beneficiaries for whom such care is specifically authorized by the Department of Veterans Affairs Medical Center. Services will be provided to eligible Veterans as requested by the Phoenix VA Health Care System (PVAHCS) within the Phoenix VA Health Care System (PVAHCS) service area (Maricopa, Gila, Pinal, and La Paz Counties). The patients to be cared for under this contract will receive these services as a substitute for institutionalization. The contracted Home Infusion Therapy Service Period of Performance shall be for one (1) base year and four (4) option years, not to exceed a duration of sixty (60) months/five (5) years; starting on or about July 1, 2014. The purpose for providing Home Infusion Services are: 1.1.to procure Home Infusion Services for the Phoenix VA Health Care System (PVAHCS) 1.2.to maintain the elder veteran at the highest level of functioning possible; 1.3.to improve the quality of life for the participants among their peers; and 1.4.to provide support for the family and other caregivers of elder veterans. 2.The care provided shall include: 2.1.Antimicrobial Infusion Therapy, 2.2.Total Parenteral Nutrition (TPN) Therapy, 2.3.Pain Management therapy, 2.4.Chemotherapy, 2.5.Hydration Therapy and Inotropic Therapy. 2.6.Capability of twenty four (24) hours, 365 days per year, nursing and pharmaceutical coverage for the patient including emergency services for replacement and/or back-up equipment/supplies, and 2.7.Transportation. 3.The Home Infusion Therapy Services will include all services and supplies related to and including Equipment, Drugs, Patient Assessment and Monitoring and Training for patients and caregivers. 4.The Home Infusion Therapy Service Contractor shall perform the following: 4.1.Provide home infusion therapy services, as specified, in the homes of eligible Veterans who are designated by, and are living within the service area of the Phoenix VA Health Care System (PVAHCS), Phoenix, Arizona, and Veterans who are followed primarily by the Phoenix VA Health Care System (PVAHCS) but who may live outside the service area within the State of Arizona. 4.2.Provide service within 24 hours of referral. 4.3.Provide a 1-800 number available 24 hours a day, 365 days a year for patients and/or caregivers to call should problems arise. 4.4.Schedule all patient visits with the patient and/or caregiver. 4.5.Provide a trained and qualified home infusion Registered Nurse (RN) to complete the initial patient admission, assessment and reassessment as determined by patient needs. 4.6.Provide clinical pharmacy consultative and monitoring service. 4.7.Provide clinical nutrition consultative and monitoring service. 4.8.Provide patient/caregiver education in the mechanics of all equipment and procedures prior to discharge from the acute care setting and as necessary in the home environment. Instruction given to the patient/caregiver will include at a minimum: 4.8.1.Safe and appropriate use of the medical equipment and/or supplies, administration of medications and signs/symptoms of reactions. 4.8.2.Basic home safety 4.8.3. Patients' rights and responsibilities 4.8.4.When and how the patient can contact the contractor for problems and equipment maintenance or repair 4.8.5.An emergency preparedness plan to prevent interruption of services 4.8.6.Universal precautions 4.8.7.Cleaning, handling, and storage of equipment and medication 4.9.Plan care and document care according to Joint Commission Standards. 4.9.1.Document patient education. 4.10.Forward a treatment plan for review by the physician and information necessary for patient management in a timely manner. 4.11.Obtain orders from Phoenix VA Health Care System (PVAHCS) physicians for all infusion therapy services. These include orders of patient admission and discharge to the program, changes in treatment. 4.12.Draw laboratory tests according to appropriate medical standards. 4.12.1.Obtain appropriate tubes from the Carl T. Hayden VAMC (CTHVAMC). 4.12.2.Laboratory tests will be run at the Carl T. Hayden VAMC (CTHVAMC). 4.12.3.Place the name, full social security number, date, and time of blood draw on all laboratory samples at the time of the blood draw. 4.12.4.Deliver the sample to Carl T. Hayden VAMC ASAP. 4.13.Provide discharge planning in conjunction with the Phoenix VA Health Care System (PVAHCS) physician managing the home infusion therapy and the designated community health nurse. 4.14.Notify the Phoenix VA Health Care System (PVAHCS) of any problems that may arise; for example, patient and/or caregiver unwilling or unable to manage in-home infusion therapy; home environment unfit for home infusion. 4.15.Assure patient is hospitalized at the VA whenever possible, unless a life threatening emergency occurs, the closest hospital. 4.16.Notify the Phoenix VA Health Care System (PVAHCS) Community Health Nurse Coordinator within 24 hours when a patient requires hospitalization Develop and Monitor a standard operating procedure (SOP) for proper storage including space, cleanliness, temperature, etc., and disposal of supplies. 4.17.Deliver all medications and supplies and set-up equipment in the area of the patient's preference, in a condition to be used. 4.18.Maintain ongoing inventory management and delivery service to the patient's residence on a regular schedule. 4.19.Maintain equipment under a preventative maintenance plan which includes electrical safety. 4.20.Continue service when the patient relocates or travels with the service area of the Phoenix VA Health Care System (PVAHCS). 4.21.Comply with universal precautions (i.e. infection control) as required in the preparation, delivery, and storage process. 4.22.Establish an emergency preparedness plan designed to provide continuing care and support appropriate to the care provided in the event of an emergency that could result in interruption of patient services. 5.Deliverables: 5.1.Fax a list of all patients receiving home infusion services including days of services and number of nursing visits by the tenth day of each month to the ACNS, HCBS 5.2.Fax a list of discharges by the tenth day of each month to the ACNS, HCBS. 5.3.Fax a summary of Patient Satisfaction results quarterly to the ACNS, HCBS. 5.4.At the time of any incident, provide, in writing, results of investigations of patient complaints regarding service to the ACNS, HCBS. 5.5.Report any known incidents of death, accident, injury, or infection related to furnished medications, supplies or medical equipment and life-threatening equipment malfunctions or equipment recalls. 5.6.Document and immediately fax incident report to the ACNS, HCBS Maintain records of suspected line infections and report same to the Associate Chief Nurse, Home and Community Based Services (ACNS, HCBS) within 24-48 hours. 5.7.Provide documentation of on-going quality improvement outcomes related to monitoring and evaluating quality and appropriateness of patient care, resolving identified problems, and pursuing opportunities to improve patient care upon request from the ACNS, HCBS. 5.8.Provide proof of licensure and staff competencies upon request from the ACNS, HCBS. 5.9.Provide a copy of the contractors Emergency Preparedness Plan upon request from the ACNS, HCBS. 6.Full attention shall be given to motivating and educating patients to achieve and maintain independence in activities of daily living to the maximum extent possible. Every effort shall be made to keep patients ambulatory and to achieve an optimal level of self-care. 7.The Contractor shall not maintain nor provide dual or segregated patient facilities which are segregated on the basis of race, creed, color or national origin. The contractor shall neither require such segregated use by written or oral policies nor tolerate such use by local custom. The term "facilities" shall include but not be limited to rooms, wards, sections, eating areas, drinking fountains, and entrances, and other like areas. 8.The Contractor shall accept referral of and shall provide all services specified in this contract for any person determined eligible by the VA Chief Medical Director or his/her designee, regardless of the race, color, religion, sex or national origin of the person for whom such services are ordered. In addition, the Contractor warrants that subcontracting will not be resorted to as a means of circumventing this provision. 9.All medical records concerning the veteran's care in the Home Infusion Service will be readily accessible to the VA. Upon discharge or death of the patient, medical records will be retained by the Contractor for a period of at least three (3) years following termination of care. Patient records will be maintained in conformance with the Privacy Act of 1974 (5 U.S.C. 552a). A medical record shall be maintained for each patient that includes at a minimum the following: 9.1.Admission record, including assessment and diagnosis 9.2.Physician orders 9.3.Treatment plan 9.4.Progress notes 9.5.Other usual and customary information 9.6.Periodic reevaluations 9.7.Discharge Summaries 10.The beneficiary will be provided Home Infusion Therapy Service at the expense of VA for a period not in excess of that stated in the referral/Authorization form, unless an extension of the authorization is provided in writing by the placing VA facility. 11.Unless specifically excluded in this contract, the per diem rate established will include the services listed herein and will also include all services or supplies normally provided other patients by the facility without extra charge. 12.If the VA Medical Center and the Home Infusion Therapy Service provider agree that the VA shall provide services under 38 CFR 17.51(f), the agreement will be incorporated into the contract and either the per diem rate will be re-established, or direct payment made to the United States to reflect the cost of VA's contribution. 13.The Contractor shall submit a properly executed HCFA or UB-04 claim form monthly for each VA funded participant to VA Fee Services for the purpose of claims processing. 14.Absences of the patient from the Home Infusion Therapy Service Program will not be reimbursable. 15.Payment made by the Department of Veterans Affairs under this contract will constitute the total cost of Home Infusion Therapy Service provided. The Contractor shall not apply additional charges to be billed to the beneficiary or his/her family, whether by the Contractor or any third party furnishing services or supplies required for such care, unless and until specific prior authorization in writing is obtained by the Department of Veterans Affairs facility that authorized payment. B.3 QUALIFICATIONS 1.Contractors must have the capability of compounding using aseptic techniques in a laminar flow hood or equal device. 2.Contractor must be licensed as a home health agency, if Home Health Therapy Services are being provided along with Infusion Therapy. 3.Contractors must be have a valid and current (within 2 years) pharmacy license from the Arizona State Board of Pharmacy. 4.Contractor must comply with all Arizona Department of Transportation and Federal Drug Administration regulations. 5.Contractors must be covered entities as defined by CMS for HIPPA. 6.A certificate of insurance is required. 7.Personnel assigned by the Contractor to perform services covered by this contract shall be licensed in the State of Arizona of the United States. All licenses held by the personnel working on this contract shall be full and unrestricted licenses. The qualifications of such personnel shall also be subject to review by the VA Chief of Staff and approval by the VA Facility Director. 8.Contractor's employees must be licensed and have all of the competency according to state licensing regulations. These should include an annual update of the following: 8.1Job/position description 8.2Scheduled performance evaluation 8.3Initial assessment of competence in providing IV therapy 8.4On-going (at least annually) assessment of IV therapy skills 8.5Verification and copy of license, certification or registration and background checks 8.6Documentation of initial and annual training/in-service 8.7Provide coordination, supervision, and evaluation of staff and services according to Joint Commission Standards. 9.The Contractor shall be responsible for protecting the personnel furnishing services under this contract. To carry out this responsibility, the contractor shall provide the following for these personnel: 9.1General Liability 9.2Workers Compensation 9.3Professional liability insurance 9.4Health Examinations 9.5Income Tax withholding, and 9.6Social Security payments. 10.The Contractor shall assign key personnel to this contract: 10.1During the first ninety (90) calendar days of the contract, the Contractor shall make NO substitutions of key personnel unless the substitution is necessitated by illness, death, or termination of employment. The Contractor shall notify the Contracting Officer, in writing, within ten (10) business days after the occurrence of any of these events and provide the information required by paragraph 3.3 below. After the initial ninety (90) calendar day period of the contract, the Contractor shall submit the information required by paragraph 3.3 to the Contracting Officer at least ten (10) business days prior to making any permanent substitutions. 10.2The Contractor shall provide a detailed explanation of the circumstances necessitating the proposed substitutions, complete resumes and licenses for the proposed substitutes, and any additional information requested by the Contracting Officer. Proposed substitutes shall have comparable qualifications to those of the persons being replaced. The Contracting Officer will notify the Contractor within 15 calendar days after receipt of all required information of the decision on the proposed substitutes. The contract will be modified to reflect any approved changes of key personnel. 10.3For temporary substitutions where the key person will not be reporting to work for five (5) consecutive work days or more, the Contractor will provide a qualified replacement for the key person. This substitute shall have comparable qualifications to the key person. Any period exceeding two weeks will require the procedure as stated above in paragraph 3.2. 11.The parties agree that the Contractor, its employees, agents and subcontractors shall not be considered VA employees for any purpose. 12.Contractor must be able to provide coordination, supervision, and evaluation of staff and services according to Joint Commission Standards. B.4 QUALITY ASSURANCE SURVEILANCE PLAN (QASP) 1.The VA has the right to inspection of the Contractor's facility and all appurtenances by an authorized representative(s) designated by the VA to determine whether acceptable standards are maintained and that adequate care is being rendered. Duly authorized representatives of the VA will provide follow up supervision visits to veterans placed to assure the continuity of care and to assist in the veteran's transition back to the community. These visits do not substitute for nor relieve the contractor in any way of the responsibility for the daily care and medical treatment of the veteran. 2.The Contractor's Home Infusion Therapy Program will meet the VA standards as promulgated in 38 CFR Part 17.51(c) through 17.51(e). 3.The Contractor shall cooperate with VA Quality Improvement activities. The VA shall have access to the Contractor's medical records to the extent permitted by State Laws. The VA allows open practitioner-patients communications regarding appropriate treatment alternatives without penalizing the contractor for discussing medically necessary or appropriate care for the patient. 4.Annual reviews will be completed to ensure that the facility remains in good standing with Arizona Department of Health Services, facilities transportation services are reliable, facility continues to provide adequate services, and will include consideration of feedback from the VA staff conducting site visits as well as feedback from veterans and family members of participants. 5.Contractors, contractor's personnel, subcontractors and subcontractor personnel shall be subject to the same Federal laws, regulations, standards, and VA Directives and Handbooks as VA and VA personnel regarding information and information system security. 6.Information made available to the Contractor or subcontractor by the VA for the performance or administration of this contract or information developed by the Contractor, subcontractors or subcontractor personnel in performance or administration of the contract shall be used only for those purposes and shall not be used in any other way without the prior written agreement of the VA. This clause expressly limits the Contractor, subcontractors and subcontractor personnel 's rights to use data as described in Rights in Data - General, FAR 52.227-14(d) (1). 7.PVAHCS has the right to conduct unannounced annual on-site inspection of the Contractor's facilities. 8.The Contracting Officer's Representative (COR) may suggest periodic changes in the competency process and review requirement in order to maintain compliance with current Joint Commission standards and other regulatory/accrediting agencies. NOTE: The purpose of this notice is to gain knowledge of potential qualified sources and their size classification//socioeconomic status (Service-Disabled Veteran Owned Small Business, Veteran Owned Small Business, HUB Zone, 8(a), Small Business or Large Business), relative to NAICS 621610.
