Description
This is not a Pre-solicitation or Solicitation announcement. This is a sources sought notice only. The purpose of this sources sought notice is to gain market knowledge of potential large and small businesses to include small business sources and their size classifications; HUBZone, 8(a), Small Disadvantage Business, Woman Owned Business, Veteran Owned Business, Service Disabled Veteran Owned Business relative to NAICS Code 621999, with a small business size standard not to exceed $10,000,000.00 gross annual receipts. Large businesses are encouraged to respond to this Sources Sought as well. Responses to this sources sought synopsis will be used by the Government to make appropriate determinations about potential sources only. No solicitation is currently available at this time. After review of the responses to this sources sought announcement, a synopsis and/or solicitation announcement may be published at a later time. Description of Services: Cardiovascular Perfusionist to provide services for perfusion cases and 24 hours a day, 365 days a year on-call services for the Saint Louis VAMC. Below is the number of cases for the procedures that are estimated at the St. Louis VAMC. In the questions below, please provide the business practice/industry standard for the fee service schedule. Cardiovascular Perfusionist Services Number of Cases 1. Complete Pump Case 100 2. Cell Saver 125 3. Stand-BY Emergencies at VAMC 20 4. IABP Setup 15 5. IABP Monitoring15 6. Extracorporeal Membrane Oxygenation (ECMO)1 7. Insertion of ventricular assist device; extracorporeal, single ventricle1 8. Insertion of ventricular assist device; extracorporeal, biventricular1 On-Call Services 24 hours/day, 7 days/weekTotal Hours per Year 8760 Responses to this notice shall include company name, address, and point of contact, contact information to include phone and e-mail address, and size of business pursuant to North American Industrial Classification Code (NAICS). Also, please respond to the following questions: Q1) How many perfusionists are typically on a case? A1) Q2) Is your business practice to charge on an hourly basis or a fee for service schedule, for the procedures? A2) Q3) If your business practice is a fee for service schedule, could you provide the typical fee schedule? (The Government is not requesting rates. For examples include: Basic Perfusion, Intra Aortic Ballon Pump Insertion and Daily Monitoring, Cell Saver, Perfusion Stand By. The Government is seeking industry standard.) A3) Q4) What is the industry standard for on-call services? Is on-call services included in the fee structure? How many perfusionists is on-call? If the perfusionist is called into the facility, is there another "On-Call Team" available for another emergency? A4) Q5) Is your business a small business? A5) Q6) What is your NAICS Code? A6) Q7) Does your firm qualify as a small disadvantaged business? A7) Q8) Is your business a large business? A8) This is not request for proposal and in no way obligates the Government to award any contract.
