- THIS IS A REQUEST FOR INFORMATION. The Social Security Administration is seeking sources capable of providing medical consultant services in a variety of specialties for work under a Blanket Purchase Agreement (BPA). The services involve the review of medical evidence in disability claims filed under Title II and XVI of SSA's Disability Programs. No patient contact will be involved. Basic computer proficiency is necessary to complete forms, read and answer email messages, and type narrative consultations. The services will be performed at the Harold Washington Social Security Administration, 600 West Madison, Chicago, IL 60661. The associated North American Industrial Classification Code for Physicians Offices (except Mental Health) is 621111, with a small business standard $10 million and for Physicians Offices (Mental Health) 621112, small business standard $10 million. Prospective contractors shall be registered in the System for Award Management (SAM) database at w! ww.sam.gov prior to award of the contract. A fixed price contract is anticipated. Contract period will be a base period of one year, with one one-year option period. This requirement is a 100% Small Business Set-Aside. Information supplied in reply to this notice will be used solely for market research purposes. This notice does not constitute a solicitation for proposal. Information supplied in reply to this notice will not constitute a proposal or an offer for services. Information submitted in reply to this notice shall be provided at no cost to the Government. This information will be used in SSA's identification of capable sources. Do not call regarding this sources sought notice.
