- Description of Services The contractor shall provide services to Veterans for whom such care is specifically authorized by the Department of Veterans Affairs, VA Medical Center, 3600 30th Street, Des Moines, Iowa 50310, herein after referred to as the (VA). The contractor shall provide after hours, weekend and holiday supportive services under the CRRC Program for the period of date of award through 1 year, with 3 option years. This is inclusive of all services as may be necessary in the care of the Veteran including laundry, showers, day room, and hygiene items as outlined in this solicitation. Unless specifically excluded in this contract, the per diem rate established will include the services listed in this document and will also include all services normally provided to other individuals by the facility without extra charge. The contractor's facility shall be located within 10 miles of the VA, within Polk County. The delineated area is in downtown Des Moines, Iowa; with optimal location close to locations where the homeless population reside. Special requirements regarding location include compatibility to surrounding land usage, degree of access to the nearest public transportation, location of the space to homeless services, homeless camps and congregation points. The contractor shall furnish services to Veterans from whom such care is authorized by the VA. It is understood that the participants of the CRRC cared for under this contract will normally require care and services over and above the non-homeless population. Many of these Veterans who are homeless or who are at risk of homelessness have co-morbid conditions that include mental illness, substance use disorders, and/or medical conditions. This contract is designed to provide homeless Veterans, or at risk Veterans in need of shelter from the weather; laundry, showers, hygiene items and a supportive environment. Work will be performed at the contractor's facility in Des Moines, Iowa. Government furnished workspace will not be provided for this effort.
