Description
Statement of Need Non-Emergent Ambulance/Van Transport Services Sayre Community-Based Outpatient Clinic Need is for non-emergent ambulance and wheel chair accessible van services to provide patient transportation from the Sayre Community-Based Outpatient Clinic to Wilkes-Barre VA Medical Center, 1111 East End Boulevard, Wilkes-Barre, PA 18711 or Robert Packer Hospital, One Guthrie Square, Sayre, PA 18840. Services are needed 5 days a week, Monday through Friday, during the hours of 7:00am to 5:00pm, and must be performed in accordance with any applicable US Department of Transportation or OSHA regulations, as well as applicable Pennsylvania State EMS Codified Laws and standard industry practice. Contractor must ensure that the total distance traveled will result in the most economical charge to the Government. Usually, only one (1) patient will be transported per trip. There may be infrequent occasions when more than one (1) patient and/or caregiver, escort is transported along with the patient. The Sayre Community-Based Outpatient Clinic may use Contractor or an alternate means for Emergent Ambulance Services as determined by appropriate Sayre Community-Based Outpatient Clinic staff. Need for non-Emergent services shall be determined by appropriate Sayre Community-Based Outpatient Clinic staff prior to patient transport. Contractor shall be required to commence transportation of the patient within ten minutes of a pre-arranged or scheduled time for patient transport. Otherwise, the Contractor shall commence transportation of the patient within forty-five (45) minutes of notification from the authorized Government Employee. In the event ambulance service is requested by the Sayre Community-Based Outpatient Clinic and the Contractor advises that they cannot begin transportation within the forty-five (45) minute response time, Sayre Community-Based Outpatient Clinic may contact another ambulance service to provide transportation. All resulting expenses will be billed to the Contractor via written notification with a bill of collection.
