- Statement of Work This is a combined solicitation/synopsis for commercial items with the intent to award IAW FAR 13.106-1(b)(1) on a sole source basis to Central Admixture Pharmacy Services, Inc. (hereafter CAPS) at: 18012 Cowman Street, Suite 250, Irvine, CA 92614. This sole source is based on CAPS being the sole local source of the required intravenous Total Parental Nutritional (TPN) customized and delivered daily for critically ill patients, as required by the Government. This solicitation is set forth as an unrestricted local set aside. Local distribution is required due to perishable customized medications and supplements. All responsible sources may submit a capability statement, proposal or quotation. All bids will be considered by the Agency. It is however the sole discretion of the Government not to compete based upon the responses to this notice. Normally, information received will only be considered for the purpose of determining whether to conduct a competitive procurement. Paper copies of this solicitation will not be available. The closing date for submitting proposals is January 30, 2014 at 10:00AM, Pacific Standard Time. This announcement constitutes the only request for quotations. No further solicitation will be issued. The solicitation number is N00259-14-T-0079. This solicitation incorporates provisions and clauses that are in effect through FAC 2005-77. It is the contractor's responsibility to be familiar with the applicable clauses and provisions. The clauses may be accessed in full text at the following web address: http://acquisition.gov. The North American Industry Classification System (NAICS) Code for this acquisition is 424210: Size: 100. All interested Offerors shall submit quotations electronically by email to sandra.kretschmar@med.navy.mil or by facsimile at 619-532-5596, ATTN: Sandra Kretschmar. E-mail submissions are limited to 2MB. The submitter should confirm receipt of facsimile and email submissions. Quotations MUST be received on or before January 30, 2014 at 10:00AM, Pacific Standard Time to be considered. All other offers shall be considered non-responsive. Naval Medical Center San Diego requests responses from qualified sources capable of providing service maintenance of the following items: Daily customized (both medication and supplements for each requiring patient) TPN. The following lists salient characteristics that must be met: 1. Requirement will be transmitted electronically daily and contents of said medical requirements are subject to the strictest of confidence. Compliance with all HIPPAA regulations is required. 2. Responsive Vendors must be able to obtain medication and supplements at times when there is a high demand and back orders exist. Patients' lives are dependent on this requirement being met. 3. Pharmacists must be fully certified and all conditions contained within the statement of work must be met. 4. Deliveries of accurate medication and supplements for intravenous distribution must be made daily before 1:00pm. The following factors shall be used to evaluate offers: 1) Technical capability (adherence to salient characteristics as described herein); 2) Delivery Schedule (requirement date is 01 February 2014); and 4) Price. When combined, technical capability and Delivery outweigh "price" as an evaluation factor. Other qualifications that must be met in order to be considered: 1. The Government will only consider firm fixed-price quotations. 2. Offerors must complete and submit quotations IAW FAR provision 52.212-3. 3. Offerors must be registered in the System for Award Managment (hereafter SAM) prior to all awards. The website address for SAM is www.sam.gov. 4. A Data Universal Number System (DUNS) number and cage code is required to register.
