Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice and in conjunction with FAR Part 13. This is a Brand Name or equal requirement. Brand name is Synthes. This procurement is for new instruments only; no remanufactured or "gray market" items. All items must be covered by the manufacturer's warranty. The solicitation number is VA248-14-Q-0432 and is issued as a Request for Quote (RFQ), unless otherwise indicated herein. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-69. The associated North American Industrial Classification System (NAICS) code for this procurement is 339112 with a small business size standard of 500 employees. This requirement is a 100% Small Business set-aside and only qualified offerors may submit bids. FOB Destination shall be Orlando, FL 32803-8208. The product is expected to be delivered no later than February 31, 2014. Orlando VA Medical Center - Orlando, Florida requires the following: Titanium Cannulated Screws (3.0mm/4.5mm) **See attached spreadsheet (EXHIBIT A-TI CANNULATED SCREWS PRICE LIST) for quantities, sizes and manufacturer part numbers. ** This announcement constitutes the only solicitation; a formal, written solicitation will not be issued. PAPER COPIES OF THIS SOLICITATION WILL NOT BE AVAILABLE. This solicitation documents and incorporates provisions and clauses in effect through FAC 2005-69. It is the responsibility of the contractor to be familiar with the applicable clauses and provisions. Quotes will be evaluated for technical acceptability and price. Technical acceptability will be determined in accordance with the salient characteristics provided. If an equal product is quoted, the product will be deemed technically acceptable if it meets or exceeds the salient characteristics listed. Failure to meet any one of the salient characteristics may deem the product to be not technically acceptable. Those quotes that do not fulfill the requirement in its entirety will be considered not technically acceptable.
