- Questions and Answers (i) This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in subpart 12.6 of the FAR and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a separate written solicitation will not be issued. (ii) Solicitation number IHS-14-HQ-RFQ-00007 applies, and is issued as a Request for Quote (RFQ). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-71. (iv) The North American Industry Classification System (NAICS) code is 541611 with a size standard of $14M. The Indian Health Service (IHS), Division of Acquisition Policy intends to solicit a fixed priced purchase order to Small Businesses only. (v) The Contract Line Items (CLINs) and pricing structure are as shown below. CLIN 0001~0004 (Base Period - March 1, 2014 ~ February 28, 2015) - Consultant Support Services for the IHS Patient & Community Needs Assessment for Care Transitions & Coordination. *Fill out the attachment 1, Schedule of Prices. (vi) The contractor will analyze resources to reduce hospital inpatient readmissions. Each of the 28 hospitals and respective tribal community served will be evaluated on the basis of patient needs in order to identify additional health care services needed by IHS patients discharged from the hospital. See the attachment 3, Performance Work Statement for details. (vii) The delivery and acceptance term for this order is FOB Destination. (viii) The provision at 52.212-1, Instructions to Offerors - Commercial Items (Jul 2013), applies to this acquisition and a statement regarding any addenda to the provision (ix) 52.212-2, Evaluation - Commercial Items (Jan 1999) (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Technical Evaluation and Price. See the attachment 2, Evaluation Factors for Award. (x) Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items (Nov 2013), with its offeror.
