Description

The Department of Veteran Affairs, NCA 8 NF/SG is seeking responsible sources to provide service as outlined in the Statement of Work. This is market research for informational and planning purposes only. This is not a solicitation and does not obligate the government to issue a solicitation. This is to gain knowledge of potential sources related to NAICS 811219. Please be advised that the Government will not pay for any information submitted under this announcement. Your response should identify your company name, point of contact with contact information, indicate all areas you are capable of providing service, capabilities statement/certification and identify your business size, and if you are any of the following: Federal Supply Schedule holders, Service Disabled Veteran Owned Small Business, Veteran-Owned Small Business, Small Business, HUB Zone certified, 8(a) disadvantaged, disadvantaged, and woman-owned. Responses must be received in writing no later than 12:00 p.m. EDT Jan 24, 2014. Responses should clearly state the ability & documentation to provide the service. If a solicitation is issued, it will be announced at a later date, and all interested parties must respond to that solicitation announcement separately. STATEMENT OF WORK Background: The Malcom Randall VA Medical Center, Gainesville, FL is requesting an annual service contract on the St. Jude Medical EnSite Velocity Cardiac Mapping system, serial number 14052734. This system creates non-fluoroscopic 3d images of the heart chamber and provides diagnostic tools which represent the anatomy of the cardiac chamber & electroanatomical information. The unit is utilized in the Cardiac Cath Labs. Objective: This is a full service contract. "Full Service" means service assistance to resolve a malfunction, such assistance to be provided either by telephone, on-site visit by an authorized St. Jude Medical technical representative, or service to be provided at an authorized St. Jude Medical service center. This support and maintenance agreement includes the repair or replacement of any or all parts of the system, a loaner system if Gainesville's is down for an extended period of time, on-site tech support, guaranteed rapid response (72 hrs.) of On-Site support, preventative maintenance, and all software upgrades. St. Jude Medical or authorized distributor will provide technical support for the maintenance and service of the EnSite Velocity System during Monday through Friday, between the hours of 8:30 a.m. EST and 5:00 p.m. EST.
