- Please see the attached Statement of Work REQUEST FOR INFORMATION Blood Pressure Measuring and Monitoring Machine I. INTRODUCTION The Office of Support Operations (OSO) of the Library of Congress (LOC) provides essential support operations and oversees statutory and regulatory compliance in the areas of human capital, administration, facilities, asset management and protection, personnel security and safety. In supporting the Library strategic goal of "Managing proactively for productive results," the Health Services Office (HSO) of OSO with the mission of promoting and maintaining optimal health, wellness and productivity of all Library of Congress staff. II. OBJECTIVES OSO is issuing this combined Sources Sought - Request for Information (hereafter RFI) to solicit input and comment on "American Disability Act compliant Self-attended Blood Pressure Monitoring Machines" to include those which are also able to measure weight. The goal of this RFI is to understand the full breath of ADA compliant blood pressure monitoring machines available, the relative order of magnitude (ROM) of cost for the various models, sources, and the standard industry lease, maintenance, and repair response terms and conditions. A draft Statement of Work that provides initial specifications is attached. Vendors are encouraged to provide comments on these specifications. Machine(s) will be located at the LOC primary campus on Capitol Hill, Washington DC, LOC annex located in Northwest, Washington DC, and the LOC campus located in Culpeper, VA. All interested and qualified businesses are encouraged to respond to this notice in accordance with the instructions addressed herein. III. INDUSTRY REPONSE A. COVER LETTER (1 page) The cover letter must clearly identify the following information: 1. Company's Name 2. Company's Address 3. DUNS Number 4. Point of Contact(s) (Name title, email, and phone number) 5. Identify your business size and business category (i.e. small disadvantaged, 8(a) certified, etc. B. CONTRACTOR'S CAPABILITY STATEMENT (not to exceed 5 pages) Respondents must have, thorough narrative description, identify their firm's capabilities to meet the requirements identified in the RFI, and experience in providing similar products to other Federal Agencies. When identifying current or past experience, please identify the following: 1. Name of Agency 2. Point of Contact (name, title, phone and email) 3. Contract/Task Order Number 4. Contract Type 5. Period of Performance 6. Total Dollar Value 7. Scope of the Contract C. White Paper (not to exceed 10 pages) OSO is interested in hearing industry's ideas on how to efficiently and effectively structure a contractual relationship. Vendors are encouraged to comment on the areas identified under Section II Objectives of this RFI. The Vendor may also provide recommendations on acquisition strategy, contract type, and a recommended Statement of Work. The White Paper shall not exceed 10 pages. Sample terms and conditions may be attached and shall not count against the page limit. IV. SUBMISSION OF INFORMATION The Capabilities Statement and White Paper must be submitted in writing. The vendor is not required to answer or comment on all content contained in this RFI in order to respond to this notice. Questions about this announcement should be directed to the Contracting Officer. Responses are due by 3:30 pm on January 24, 2014. V. OTHER Information received on the RFI may be used to assist the Library in creating an Request For Proposal and may be shared with other Federal agencies. This RFI is for information and planning purposes only and does not constitute a solicitation. All information received in response to the RFI that is marked PROPIETARY will be handled accordingly. The Government will not return or pay for any information provided in response to this announcement; no basis for a claim against the Government shall arise as a result from a response to this notice or Government use of any information provided. Respondents are solely responsible for all expenses associated with responding to this announcement. TERMS AND CONDITIONS: 52.215-3 Request for Information or Solicitation for Planning Purposes (Oct 1997) a) The Government does not intend to award a contract on the basis of this solicitation or to otherwise pay for the information solicited except as an allowable cost under other contracts as provided in subsection 31.205-18, Bid and proposal costs, of the Federal Acquisition Regulation. (b) Although "proposal" and "offeror" are used in this Request for Information, your response will be treated as information only. It shall not be used as a proposal. (c) This solicitation is issued for the purpose of gathering information on self-attended ADA compliant blood pressure and weight and blood pressure machines.
