- Attachment 4 - Government Furnished Property Listing Attachment 3 - DFARS 252.209-7994 Attachment 2 - Provisions and Clauses Attachment 1 - Performance Work Statement Solicitation FA8601-14-T-0034 COMBINED SYNOPSIS/SOLICITATION This is a combined synopsis/solicitation ("synopsitation" hereafter) for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Government reserves the right to award without discussions or make no award at all depending upon the quality and price reasonableness of offers received. An award, if any, will be made to the responsible offeror whose proposal conforms to the requirements of the synopsitation and represents the best value to the Government, price and other factors considered. Synopsitation FA8601-14-T-0034, is a Request for Proposals (RFP) and incorporates FAR provisions and clauses in effect through Federal Acquisition Circular (FAC) 2005-71, effective 25 Nov 2013; Defense Federal Acquisition Regulation Supplement (DFARS) Publication Notice (DPN) 20131206, effective 06 Dec 2013; and Air Force Federal Acquisition Regulation Supplement (AFFARS) Air Force Acquisition Circular (AFAC) 2013-0827, effective 27 Aug 2013 The NAICS code for this acquisition is 621399 and the associated small business size standard is: $7 Million. This RFP is a 100% total small business set-aside. This requirement is for 1 FTE Medical Patient Transporter per the attached specifications. IMPORTANT NOTICE TO CONTRACTORS: All prospective awardees are required to register at the System for Award Management (SAM) and to maintain active registration during the life of the contract. SAM can be accessed at https://www.sam.gov. Any award resulting from this solicitation will include DFARS Clause 252.232-7003, Electronic Submission of Payment Requests. Section 1008 of the National Defense Authorization Act of Fiscal Year 2001 requires any claims for payment (invoices) under DoD contract to be submitted in electronic form. Wide Area Workflow- Receipt and Acceptance (WAWF-RA) is the DoD system of choice for implementing this statutory requirement. Use of the basic system is at no cost to the contractor. Contractors must complete vendor training, which is also available at no cost at http://www.wawftraining.com. Prior to submitting invoices in the production system, contractors must register for an account at http://wawf.eb.mil/. Contract financing is NOT provided for this acquisition. The Government intends to award one firm fixed price (FFP) contract resulting from this solicitation to the responsible offeror whose proposal conforms to the solicitation and will be most advantageous to the Government. The Government intends to award without discussions and the Government reserves the right not to make an award at all. Consent to RFP Terms and Conditions: Submission of proposals in response to this RFP constitutes agreement by the offeror to all terms and conditions contained in this announcement and the attached solicitation document, which will be incorporated into the resulting contract. Contract Line Items (CLIN): This RFP has four (4) CLINs. Detailed descriptions of these CLINs are located in the Provisions and Clauses (Attachment 2). Period of Performance (Estimated, PoP starts day of award) Base Year 30 January 14 - 29 January 15 Option Year I 30 January 15 - 29 January 16 Option Year II 30 January 16 - 29 January 17 Option Year III 30 January 17 - 29 January 18 Six- Month Extension 30 January 18 - 29 July 18 Delivery Destination: Wright-Patterson Medical Center 881 Sugar Maple Drive Building 830 Wright-Patterson AFB, OH 45433-5529 FOB: Destination (As defined in FAR 2.101-Definitions, the seller or consignor is responsible for the cost of shipping and risk of loss.)
