Description
STATEMENT OF WORK The Naval Health Research Center (NHRC) has a requirement for (1) transportation of office materials, furniture, equipment and documents or files from buildings within NHRC and/or to the Defense Reutilization Management Office (DRMO) or the Recycling Center or other specified locations. The company may be required to disassemble / reassemble and make adjustments to various types of furniture and equipment in order to facilitate the transportation to final location. MOVING SERVICES ARE GENERALLY REQUIRED ABOUT 4 TO 5 TIMES ANNUALLY, BUT THE COMPANY SHALL BE "ON CALL" AS NEEDS ARISE THROUGHOUT THE FISCAL YEAR. 1. Scope of Work For each job, the vendor will supply the following services: Disassembly, moving, staging, lading, transportation and/or reassembly of office furniture and/or laboratory or medical equipment identified from any of the buildings located at NHRC to specified locations within or outside of Naval Base Point Loma (NBPL). These locations may include NHRC owned facilities at NBPL in the barracks area: Buildings 304, 309, 310, 314, 315, 320, 322, 328, 329, 330, 331, 332, 333, 335, 346, 347, and 434. Other NHRC locations include, the Fleet Combat Training Center Complex Fleet Combat Training Center, Norman Scott Road, San Diego, CA 92136-5588, building 62 and 74. Other moving requirements may include: Collection and transport of serviceable furniture and equipment identified for DRMO to the U. S. Reuse Furniture Warehouse on Naval Base San Diego in the NEX warehouse complex at 401 West 8th Street San Diego Ca, 91950. Collection and transport of unserviceable scrap metals to the NBPL Recycling Center located in bldg 379 on McClelland Road, 1/8 of a mile from NHRC and the barracks area, or to specified refuse receptacles. Care must be taken to separate metal component from unserviceable furniture and equipment to take full advantage of recycling. Therefore, chairs, desks, partitions and other furniture will be disassembled to remove metal components (where feasible) to collect metal components. Equipment that is inherently metal may not require disassembly and can be collected with other metal components. Nonmetal components such as plastic and wood, once disassembled from the "furniture" items may be discarded in to the refuse container located in the barracks area compound. - Disassembly and relocation of furniture or equipment for movement between NHRC owned buildings with 1st and 2nd floors. - Disposal of other items deemed unusable into trash receptacles designated by NHRC personnel. Some disassembly may be required to separate recyclable metals from scrap. - Vendor is to maintain and assert an Acceptable Quality Level Standard, that is, a system that ensures the offices are cleared of all materials that are marked DRMO, or that are to be recycled. Furniture identified for DRMO will be packaged by the vendor, to ensure 0% damage, and delivered to Furniture Reuse Center at the San Diego Broadway Navy Complex 937 North Harbor Drive, San Diego. - Estimating 165 hours TOTAL for Truck & Driver during Period of Performance: 12 months from contract award date. - Estimating 332 hours TOTAL for the Material Handler during Period of Performance: 12 months from contract award date. - All work is to be completed during normal business hours 0700 - 1630 Monday thru Friday. Transportation from vendor location to NHRC and back to vendor location is not covered under the contract. - Other situations where lifting of heavy equipment and furniture is required. - Contractor will only take requests from the Contracting Officer Representative (COR) or the point of contact (POC) designated by NHRC. 2. Services The Contractor will perform the following moving services in the highest commercial quality and industry standards. Origin Services - Disassembly of furniture and/or equipment: - Loading of furniture and/or equipment at point origin. Transportation Services - Secure transportation of furniture and/or equipment utilizing company vehicles and equipment. Destination Services - Unloading of furniture and equipment at destination. - Assembly and reassembly of furniture pieces at the destination. - Making adjustments to furniture assembly as needed. 3. Equipment - Contractor shall provide the truck and driver to perform the services in a professional manner. - Contractor shall also perform the services in a first class manner. All costs for equipment and fuels shall be incorporated in the purchase price proposed below. 4. Safety - Contract is responsible for adhering to all Occupational Safety and Health Administration (OSHA) standards pertaining to the moving of equipment by their personnel. Safety harnesses, belts, back supports, goggles, gloves, and other personal safety equipment is required at all times. 5. Clearance - Contractor is responsible for gaining access to NBPL and other Naval Base sites and must be cleared prior to work beginning. Contractor personnel should be easily identifiable (i.e. name tags, badges et) in order to insure that personnel removing equipment from NHRC facilities are authorized to do so. The contractor is responsible for providing transportation to their personnel to and from the job site in company vehicles. No privately owned vehicles (POV) of the contractor's personnel will be afforded access to NBPL unless the vehicle has a valid military base access sticker. Changes to personnel list must be submitted in writing prior to access being granted. 6. Check-In - Contractor will be notified of upcoming moves and DRMO requirements no less than one week prior to the requirement. Emergencies will be handled on a case by case basis, and are based upon contractor availability. The contractor will check in with pre-designated personnel from the NHRC Operations Department prior to beginning work. The contractor will check out with pre-designated personnel from the NHRC Operations Department prior to departing NBPL. Vendor will only take direction from designated personnel from the NHRC operations department. The time allotted for performance of work must be accounted for in writing on a document provided by the contractor. Essential elements of this document are: 1. Number of Personnel 2. Hours Worked 3.
