- 1. Issue Date: 01/06/2014 Close Date: 01/21/2014 1200 EST Please refer to the attached solicitation, W81XWH-14-T-0073. 2. Requiring Activity: US Army Medical Research Acquisition Activity (USAMRAA) for the US Army Center for Environmental Health Research (USACEHR). 3. Description of Requirement: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. 4. Solicitation Number: W81XWH-14-T-0073 This combined synopsis/solicitation is issued as a Request for Quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-72, 01 January 2014. This combined synopsis/solicitation is a 100% small business set-aside procurement using North American Industry Classification System (NAICS) Code 334516. Small Business size standard for this NAICS code is five hundred (500) employees. The USACEHR requires two devices for performing testing of blood samples from rodents. One device must be a bench top system and the other must be a handheld/portable device with compatible printer. Both systems must utilize disposable cartridge style assays with multiple tests on each cartridge. All quotations shall satisfy and comply with the Minimal Essential Characteristics (MEC's) which are provided in the Statement of Need (SON). All quotations shall provide a matrix that directs a point back to all MEC's. Quotations shall include all costs associated with the Delivery Schedule, Installation, Set-up, Training, and Warranty. Contract shall be Firm Fixed-Price. All quotations shall be evaluated in accordance with FAR 52.212-2. Quotations shall be prepared in accordance with FAR 52.212-1. Additional Instructions to Offerors Due to the difficulties in receipt of mail through normal postal services, quotations shall be sent via e-mail to the POC for this combined synopsis/solicitation, to roman.e.laterza.civ@mail.mil. The Offeror is responsible for ensuring receipt by the POC. Questions shall be submitted to the Point of Contact (POC); Roman Laterza at roman.e.laterza.civ@mail.mil, no later than 14 January 2014 1200 EST. Follow-up additional questions will not be accepted after this date. Questions and answers will be provided via an amendment to this combined synopsis/solicitation. Quotations are due by 21 January 2014 1200 EST. Quotations shall provide a price for ALL items listed. Your submission should include two separate parts: a Price proposal and a Non-Price proposal. The Non-Price proposal shall be submitted in MS Word or searchable PDF format. The Price proposal shall be submitted in MS Word or MS Excel 2000 or higher, but not greater than MS Word or MS Excel 2003 format. The firewall utilized by the Government contracting office does not authorize or accept zip files. Quotations shall include all delivery charges for products specified in the vendor's quotation. Requested delivery date can be found in the Delivery Schedule of the solicitation. If requested ship dates cannot be met, please comment on estimated ship/delivery date. Please include CAGE Code and DUNS number. Contractors shall be aware of the difference between Not Separately Priced (NSP) versus Zero. NSP means the Government is being charged for this service or supply; however it is included in the overall price and not separately priced. Zero means the Government is not being charged for this service or supply; however the Government is still receiving this service or supply for zero dollars.
