Description
Scope of Work The Louis A. Johnson VAMC Pharmacy Service is looking for a vendor to provide nationally recognized credentialing of our pharmacist residency, post-graduate year 1 (PGY-1) program effective January 1, 2014. This accreditation must include a thorough review of the pharmacy residency program established at the facility. Vendor must include an established criteria for systematic training of pharmacists for the purpose of achieving professional competence in the delivery of patient-centered care and in pharmacy operational services. Criteria for accreditation must delineate the requirements for the PGY1 residency that builds upon the educational foundation provided through completion of an accredited Doctor of Pharmacy degree program. DEPARTMENT OF VETERANS AFFAIRS Justification and Approval For Other Than Full and Open Competition 1.Contracting Activjty: Department of Veterans Affairs, VISN 04, Louis A. Johnson VA Medical Center 540-14-1-036-0118 2.Nature and/or Description of the Action Being Processed: Sole Source Justification for a firmed-fixed price task order for the annual accreditation fee for the Residency 1 Program for the Pharmacy. 3.Description of Supplies/Services Required to Meet the Agency's Needs: The Louis A. Johnson VAMC Pharmacy Service is looking for a vendor to provide nationally recognized credentialing of our pharmacist residency, post-graduate year 1 (PGY-1) program effective January 1, 2014. 4.Statutory Authority Permitting Other than Full and Open Competition: 41 USC §253(g), as implemented by FAR 6.302. The specific paragraph must be cited. Most likely, only the -1 or -2 authorities will be cited. Please be aware that failure to adequately plan for future requirements does not constitute appropriate use of the unusual and compelling urgency statutory authority. (X)(1) Only One Responsible Source and No Other Supplies or Services Will Satisfy Agency Requirements per FAR 6.302-1; ( ) (2) Unusual and Compelling Urgency per FAR 6.302-2; ( ) (3) Industrial Mobilization, Engineering, Developmental or Research Capability or Expert Services per FAR 6.302-3; ( ) (4) International Agreement per FAR 6.302-4 ( ) (5) Authorized or Required by Statute FAR 6.302-5; ( ) (6) National Security per FAR 6.302-6; ( ) (7) Public Interest per FAR 6.302-7; 5.Demonstration that the Contractor's Unique Qualifications or Nature of the Acquisition Requires the Use of the Authority Cited Above (applicability of authority): ASHP is the only nationally recognized non-governmental, non-profit pharmacy association that has been accrediting pharmacy residencies since 1962 and pharmacy technician training programs in the United States since 1983. 6.Description of efforts made to ensure that offers are solicited from as many potential sources as deemed practicable: A Justification and Approval (J&A) will be posted to Federal Business Opportunities (FBO) to determine if there are any contractors interested and capable of providing the services requested for this procurement which may not have been identified by market research. 7.Determination by the Contracting Officer that the Anticipated Cost to the Government will be Fair and Reasonable: The cost is fair and reasonable. The annual fee has increased 5% each year since the facility started the program. This year's fee is $3500. 8.Description of the market research conducted and the results, or a statement of the reasons market research was not conducted: Only one responsible source and no other supplies or services will satisfy agency requirements per FAR 6.302-1. 9.Any other facts supporting the use of other than Full and Open Competition: None. 10.Listing of sources that expressed, in writing, an interest in the acquisition: None. 11.A statement of the actions, if any, the Agency may take to remove or overcome any barriers to competition before making subsequent acquisitions for the supplies or services required: Facility attempted to acquire a 5 year contract with the contractor, but contractor could not provide pricing requested to effectively solicit and award any potential contracts to interested parties.
