Description
Note: This notice is published in accordance with Federal Acquisition Part (FAR) Part 10, Market Research and FAR Part 15, Exchanges with Industry before Receipt of Proposal. This notice serves as a Request for Information (RFI) in support of market research and/or planning purposes only and does not constitute a solicitation. This notice is not a confirmation that the Government will contract for the items contained in the RFI, nor is any commitment on the part of the Government to award a contract implied. This notice does NOT constitute an Invitation to Bid, a Request for Proposal, or a Request for Quote. The Government shall not pay and/or reimburse respondents and/or associates for any costs incurred to submit the requested information. All information received in response to this RFI that is marked Proprietary will be handled accordingly. In accordance with FAR 15.201(c), response to this RFI is not offers and cannot be accepted by the Government to form a binding contract. Do you currently hold a government contract (local/state/federal government)? Please provide contract number and contract type. 5. Current or prior experience at any VA Medical Centers? If so, where? List VA point of contact, telephone number, and email address. 6. Available to provide services in Erie, PA? Timeframe required to initiate service? 7. Experience providing similar services? a. # of years of experience b. history of conformance to specifications and standards of good workmanship c. history of reasonable and cooperative behavior and commitment to customer satisfaction 8.Common approaches to DME services. 9.Common obstacles and risks associated with providing DME services. 10.Common performance standards and assessment methods. Draft Scope of Work The contractor shall provide all necessary storage, transportation, set up, installation, maintenance/ repair, and management of Durable Medical Equipment (DME) for the Erie VA Medical Center (Erie VAMC), 135 East 38th Street, Erie, Pennsylvania. This contract will be for an initial period of one (1) year from date of award with four (4) options to extend if successful performance results are achieved based on Erie VAMC evaluation and funding. The acquisition of DME Services is being initiated to provide these services to veteran beneficiaries serviced by the Erie VAMC. Contractor shall provide DME services to any patients that are treated at the Erie VAMC and all of its Community Based Outpatient Clinics (CBOCs) in strict accordance with the performance work statement. Areas of service for the Erie VAMC include, but are not limited to the following counties: Counties in PA: Erie, McKean, Crawford, Venango, Mercer, Warren, and Forrest Counties in OH: Ashtabula Counties in NY: Chautauqua, Cattaraugus This durable medical equipment may include, but is not limited to: hospital beds, mattresses, side rails, patient lifts, wheelchairs, scooters, and telehealth equipment. All equipment shall be furnished by the Erie VAMC. All DME services shall be performed in the veteran's place of residence or nursing home housing the veteran. The contractor shall develop and maintain an organization wide planning structure and processes that focus on safety and quality to ensure services are performed in accordance with the performance work statement. The contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. All work shall be performed in accordance with Joint Commission Home Care standards. The Government shall evaluate the contractor's performance under this contract in accordance with the Quality Assurance Surveillance Plan (QASP). The QASP defines how the performance standards will be applied, frequency of surveillance, and minimum acceptable defect rate(s). The contractor shall provide education to each veteran and their family based on the patient's needs and abilities at the time of delivery. This information is to be presented both written and verbally. The contractor shall provide each veteran of copy of VA Patient Rights and Responsibilities and any applicable information concerning advance directives during initial setup. In addition, the contractor shall provide the following reports to the Erie VAMC as necessary: Falls Report (report when a fall has occurred, injuries, possible causes, corrective action), Incident Reports (special incidents of unsafe use of DME), Customer Satisfaction Report (random sampling of 10% of contracted patients and their families to be reported on a quarterly basis), Infection Control Report (according to the contractor's Infection Control Program).
