THIS IS A SOURCES SOUGHT NOTICE ONLY - DO NOT SUBMIT PROPOSALS. The purpose of this Sources Sought announcement is to identify sources capable of providing support for this effort. The information received will be used to determine if the requirement can be set aside for small business. Responses from small businesses are highly encouraged. Based on responses to this notice, the Government may consider a small business set-aside for this requirement. The Department of Veteran Affairs, Network Contracting Office 19 has a requirement for a Cardiac & Orthopedic Rehabilitation Physical Therapist in the Salt Lake City (SLC) area or the Salt Lake City (SLC) Valley area with the following qualifications: Physical Therapy License, Advanced Cardiac Life Support (ACLS) Certification, at least 7 years cardiac rehab experience, Doctorate Degree in Physical Therapy to include teaching and mentoring experience in cardiac rehab. Services include but are not limited to the following: Serve in the capacity of a Physical Therapist providing Orthopedic and Cardiac Rehabilitation Patient care, Serve as the Cardiopulmonary Rehabilitation Specialist in the department, assist with the development of the Cardiopulmonary Rehabilitation Program for the SLC VA Medical Center (VAMC), provide exam and progress reports in patient files, provide 250 hours/.12 FTE of clinical support per year; performed primarily within the Physical Therapy Department, or in other areas within the hospital as assigned by the Clinical Manager for Rehabilitation Services. The Salt Lake City VA Medical Center will provide all the usual equipment required to perform the assigned duties. Interested business concerns, large or small, should review and submit the completed questionnaire that is attached. The anticipated NAICS Code is 621340, OFFICES OF PHYSICAL, OCCUPATIONAL AND SPEECH THERAPISTS, AND AUDIOLOGISTS with a small business size standard of $7M. The anticipated contract will contain a 12 month base period plus four (4) 12-month option periods. Interested organizations are required to identify their size standards in accordance with the Small Business Administration. The Government requests that no proprietary or confidential business data be submitted in a response to this notice. However, responses that indicate the information therein is proprietary will be properly safeguarded for Government use only. All completed questionnaires sent in response to this Sources Sought Notice must be submitted electronically (via email) to Michael Vela, Contract Specialist at michael.vela@va.gov by 4:00 PM Mountain Time, 12/31/2013. This notice does not obligate the Government to award a contract or otherwise pay for the information provided in the response. The Government reserves the right to use information provided by respondents for any purpose deemed necessary and legally appropriate. Respondents are advised that the Government is under no obligation to acknowledge receipt of the information received or provide feedback to respondents with respect to any information submitted. After review of the responses received, pre-solicitation and solicitation notices may be published in Federal Business Opportunities or VA GSA. However, responses to this notice will not be considered adequate responses to a solicitation. The solicitation release date is pending. The Government intends to negotiate a fixed-price contract.
