Description
STATEMENT OF WORK New Pharmacy Operation Suite See Attached specifications and floor plans General conditions: 1. Delivery/Freight - Delivering equipment to the job site shall be at the discretion of the contractor. Delivery also includes removal of equipment from delivery vehicle and transfer of equipment to location Colorado Springs Clinic at 3139 Centennial Blvd, Colorado Springs, CO 80907. Schedule is dependent upon coordination of construction schedule and will be finalized upon award of contract. Planned installation begins late April 2014 for 2 week installation, late finish by late May, 2014. Actual installation start date to be confirmed upon award of contract. 2. Contractor to provide a 48 hour notice of delivery to VA POC. Working hours are during business hours 7:30a - 4:30p. 3. Acceptance - Contractor's equipment order, once installed, will be verified by VA Biomedical Engineering Team that equipment is in working condition according to manufacturer's technical specifications for full payment of services. Contractor to provide a specialized personnel, for consultation and oversight for entire scope of work including, planning, installation, delivery, testing, acceptance and certification 4. Guarantee - The equipment supplier shall guarantee all material and workmanship of equipment provided under the contract for a period of one (1) year from date of final acceptance. Any defects due to use of improper materials or workmanship occurring within that time shall be promptly rectified by the equipment supplier at his own expense upon notification by the Owner or Architect. Warranty begins at time of VA acceptance of equipment/after inspection. 5. Equipment site drawings for the new location will be provided by Contractor 6. Contractor to complete at least three (3) site visits prior to installation and at least one (1) for final review, testing and subsequent acceptance by VA for the following: a. Coordination meetings with Construction Contractor and VA point of contact to review space and utility connections to complete design drawings/shop drawings b. Upon design drawing review and sign-off c. Pre-construction meeting for final review of space d. Post-construction meeting for review and acceptance
