- SINGLE SOURCE (APPLICABLE TO PURCHASES UNDER SAT $3,000 - $150,000) 1.Contracting Activity: NCO 12, GREAT LAKES ACQUISITION CENTER, 115 SOUTH 84TH STREET, SUITE 101 MILWAUKEE, WI 53214 2.Purchase Request Number: 578-13-4-116-0221 3.Authority for Action: 1 FAR 13.106-1(b), Soliciting from a Single Source 4.Describe the supply or service to be acquired and the total estimated dollar value: Firm-fixed price contract to obtain specialized content and scientific expertise and services related to assessment of patient-centered care as part of the Spinal cord Injury Quality Enhancement Research Initiative (SCI QUERI). Estimated dollar value is $50,000 for the base year and one option year. 5.List the proposed contractor (name, contact, and phone number) and describe the unique knowledge or capabilities of the proposed contractor: The proposed contractor is the Rehabilitation Institute of Chicago, 345 W. Superior St., Chicago, IL, 60611, 312-238-2802. The Rehabilitation Institute is a non-profit small business with extensive expertise in delivery, receipt, and perspectives of patient-centered care in persons with spinal cord injuries and disorders. The institute has knowledge of how different cohorts of patients and their families perceive patient-centered care, barriers to patient-centered care delivery in typically underserved populations and development of optimal strategies to improve patient-centered care delivery and outcomes for individuals with SCI/D. 6.Describe your market research efforts and efforts to ensure that offers are solicited from as many potential sources as is practicable: The Rehabilitation Institute of Chicago is one of 14 SCI model system centers across the United States recognized by the U.S. Department of Education's National Institute on Disability and Rehabilitation Research (NIDRR) as centers that work to demonstrate improved care, maintain a national database, participate in independent and collaborative research, and provide continuing education relating to spinal cord injury. The Rehabilitation Institute of Chicago is the only center in the state of Illinois recognized by the NIDRR. 7.State why you feel that the anticipated cost to the Government will be fair and reasonable: IAW FAR 13.106-3(a), price reasonableness may be shown by comparison of the proposed price with prices found reasonable on previous purchases. The proposed price for this procurement is $50,000 for 15 months. A procurement of similar services was made on contract #VA69D-13-C-014 for $80,000. Based on comparison, the proposed price is determined to be fair and reasonable. 8.Other: None
