SINGLE SOURCE (APPLICABLE TO PURCHASES UNDER SAT $3,000 - $150,000) 1.Contracting Activity: NCO 12, GREAT LAKES ACQUISITION CENTER, 115 SOUTH 84TH STREET, SUITE 101 MILWAUKEE, WI 53214 2.Purchase Request Number: 537-13-3-026-0117 3.Authority for Action: 1 FAR 13.106-1(b), Soliciting from a Single Source 4.Describe the supply or service to be acquired and the total estimated dollar value: Functional Magnetic Resonance Images (fMRI) of the brain as for the VA research study "Cognitive, Clinical, and Neural Markers of Late Life Depression." Total estimated value is $55,800.00 for one base period and two option years. 5.List the proposed contractor (name, contact, and phone number) and describe the unique knowledge or capabilities of the proposed contractor: The proposed contractor is the University of Illinois at Chicago (UIC) 3T Center for Magnetic Resonance Research, 203 Administrative Office M/C 672, 1801 w. Taylor St., Chicago, IL 60612, contact - K.Luan Phan, MD, PhD, 312-413-3979. 6.Describe your market research efforts and efforts to ensure that offers are solicited from as many potential sources as is practicable: The fMRI is a key measure for this study. The Jesse Brown VA does not have fMRI capability. The study protocol requires the use of a 3.0 T MR scanner, which Jesse Brown also does not possess. There is no other facility equipped to produce fMRIs in the Illinois Medical District, where the majority of the study will take place. It is necessary that the research subjects have access to an fMRI facility in the immediate vicinity of the Jesse Brown VA. In addition, funding has been approved and awarded by VA Central Office, by the Director of Health Services Research based on a proposal submitted by Dr. Weisenbach specifying the use of the UIC 3.0T scanner. 7.State why you feel that the anticipated cost to the Government will be fair and reasonable: A similar service was procured under contract # VA69D-12-C-0296 for a separate study at Hines. The cost under that contract was $500.00 per hour. The proposed cost for the current procurement is $475.00 per hour. IAW FAR 13.106-3(a)(2), the contracting officer determines the anticipated cost to the Government to be fair and reasonable. 8.Other: Notice of intent to negotiate a sole source contract was posted to FBO. 9.Certifications: I hereby certify that the data provided above is accurate and complete to the best of my knowledge and belief.
