- This is Amendment 0004 for solicitation FA5707-14-Q-0002 Anesthesia and Ventilator Equipment. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included within this notice. This announcement constitutes the only solicitation; information is being requested and a written solicitation will not be issued. The reference number for this procurement is FA5707-14-Q-0002 and is issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-58. The associated North American Industrial Classification System (NAICS) code for this procurement is 339112. Description: anesthesia and ventilator equipment for the 376 AEW/TSC Transit Center at Manas, Kyrgyz Republic. See attached solicitation document. Shipping and Delivery: Shipping costs (FOB Destination) shall be included in line items and date of delivery shall be by NLT 30 days ARO. Place of delivery for acceptance is Ministry of Health, 149 Moskovskaya Str, Bishkek City, Kyrgyzstan. The following FAR clauses and provisions are in this acquisition: FAR 52.212-1 Instructions to Offerors Commercial Items; Evaluation/Award: IAW FAR 52.212-2, Evaluation-Commercial items applies to this acquisition. The Government will award a single contract resulting from this solicitation to the responsive responsible offeror whose quote conforms to the solicitation and will be most advantageous to the government, price and other factors considered. The following factors shall be used to evaluate quotes: Price and delivery. Please refer to attached document for all FAR provisions and clauses applying to this solicitation. Offerors shall include completed copy of the Online Offeror Representations and Certification Application. Please refer to http://orca.bpn.gov/login.aspx for information regarding the Online Representation and Certification Application process. Quotations must be received by November 10, 2013 -4:00 PM Local Manas, Kyrgyzstan time.
