- OFFICE OF INTERAGENCY HEALTH AFFAIRS (IHA) STATEMENT OF WORK INTERIM TECHNICAL AND MODEL DEVELOPMENT ASSISTANCE DURING IMPLEMENTATION OF THE RURAL VETERANS COORDINATION PILOT 1.PURPOSE. OFFICE OF INTERAGENCY HEALTH AFFAIRS (10P5), DEPARTMENT OF VETERANS AFFAIRS - NEED FOR TEMPORARY HIGHLY TECHNICAL AND SPECIALIZED SUPPORT. 2.BACKGROUND. THE OFFICE OF INTERAGENCY HEALTH AFFAIRS IS RESPONSIBLE FOR EXECUTING THE VETERAN'S HEALTH ADMINISTRATION (VHA) FOR THE IMPLEMENTATION OF THE PILOT PROGRAM REQUIRED BY PUBLIC LAW 111-163: CAREGIVERS AND VETERAN'S HEALTH SERVICES ACT OF 2010, § 506. The Mission Of § 506: Pilot Program On Use Of Community-Based Organizations And Local And State Government Entities Is To Ensure That Veterans Receive Care And Benefits For Which They Are Eligible (Attachment A) And To Provide Rural Outreach And Coordination Of Care And Benefits To Veterans And Their Families Transitioning From Military Service. Specifically, The Secretary Of Veterans Affairs Shall Carry Out A Pilot Program To Assess The Feasibility And Advisability Of Using Community-Based Organizations And Local And State Government Entities: oTo Increase The Coordination Of Community, Local, State, And Federal Providers Of Health Care And Benefits For Veterans, To Assist Veterans Who Are Transitioning From Military Service To Civilian Life In Such Transition; oTo Increase The Availability Of High Quality Medical And Mental Health Services To Veterans Transitioning From Military Service To Civilian Life; oTo Provide Assistance To Families Of Veterans Who Are Transitioning From Military Service To Civilian Life To Help Such Families Adjust To Such Transition; And oTo Provide Outreach To Veterans And Their Families To Inform Them About The Availability Of Benefits And Connect Them With Appropriate Care And Benefit Programs. 3.Objectives and Justification. IHA has only one staff support position for this pilot. Therefore the IHA, VHA office is in critical need of temporary technical and specialized grant support. The objective of this contract is to provide interim staff support from a specialist in contacts and rural outreach and coordination of care and benefits to Veterans and their families transitioning from military service, supporting the core office. 4.Scope. The Contractor shall provide services. These services shall include supporting IHA with highly technical and specialized skills in the arena of guiding model development and training activities for a new VA initiative. 5.Mandatory Tasks and Deliverables. The contractor shall provide training to the five (5) grant award recipients about the use of the funding and quarterly reports needed by the mandates of the statute. The grant and subject matter expert primary tasks and deliverables includes: oDeveloping quantitative performance measures to assess the effectiveness and outcomes of the pilot program. oRequesting information from grantees after the completion of the term of the pilot and will assemble customer satisfaction indexes from Veterans and their families, and provide a report to Congress (as required by the authorizing legislation) on the feasibility of continuing the program beyond the pilot stage. oSeek from the grantees will be self-assessments of their effectiveness at providing the services for which they received the grants, as well as cost-benefit analyses indicating their ability to manage funds to produce the desired outcomes. A review panel within VA will review the data and rank grantees against criteria being developed for evaluation purposes. oDevelop information that will be requested from Veterans and their families will include measures of satisfaction relative to each grantee's services, including scope of care, benefit delivery and coordination. oAssess the grantees own performance in overseeing the grant program, by evaluating the process for making grants and monitoring and responding to grantees. Quantify utilization by Veterans of services offered by or through the grantees. oUse information obtained through the pilot program to evaluate the effectiveness of using community-based organizations and local and State government entities to improve the provision of services to transitioning veterans and their families. oOther duties as assigned. 5.1Task One. The contractor shall provide quantitative performance measures to assess the effectiveness and outcomes of the pilot program. Deliverable 5.1.1 Meet and train grantees concerning providing quantitative performance measures to assess the effectiveness and outcomes of the pilot program. Deliverable 5.1.2 Provide IHA with quantitative performance measures to assess the effectiveness and outcomes of the grantees pilot program. 5.2 Task Two. Requesting information from grantees after the completion of the term of the pilot and will assemble customer satisfaction indexes from Veterans and their families, and provide a report to Congress (as required by the authorizing legislation) on the feasibility of continuing the program beyond the pilot stage. Deliverable 5.2.1 - Meet and train the grantees on customer satisfaction indexes. Deliverable 5.2.2 - Advise IHA on deliverables for the customer satisfaction surveys. 5.3 Task Three. Seek from the grantees will be self-assessments of their effectiveness at providing the services for which they received the grants, as well as cost-benefit analyses indicating their ability to manage funds to produce the desired outcomes. A review panel within VA will review the data and rank grantees against criteria being developed for evaluation purposes. Deliverable 5.3.1 - Meet and train the grantees on self-assessments and cost-benefits analyses. Deliverable 5.3.2 - Advise IHA on deliverables grantees on self-assessments and cost-benefits analyses Deliverable 5.3.3 - Develop a review panel within VA will review the data and rank grantees against criteria being developed for evaluation purposes. 5.4 Task Four. Develop information that will be requested from Veterans and their families will include measures of satisfaction relative to each grantee's services, including scope of care, benefit delivery and coordination. Deliverable 5.4.1. Meet and train the grantees on information that will be requested from Veterans and their families will include measures of satisfaction relative to each grantee's services, including scope of care, benefit delivery and coordination. 5.5 Task Five - Assess the grantees own performance in overseeing the grant program, by evaluating the process for making grants and monitoring and responding to grantees. Quantify utilization by Veterans of services offered by or through the grantees. Deliverable 5.5.1. Meet and train the grantees how to properly assess the grantees own performance in overseeing the grant program, by evaluating the process for making grants and monitoring and responding to grantees. Use information obtained through the pilot program to evaluate the effectiveness of using community-based organizations and local and State government entities to improve the provision of services to transitioning veterans and their families. Deliverable 5.6.1 - Meet and train the grantees how to properly use information obtained through the pilot program to evaluate the effectiveness of using community-based organizations and local and State government entities to improve the provision of services to transitioning veterans and their families. 5.7 Task Seven - Other duties as assigned. Deliverable 5.7.1 - In the meetings and teachings the grantees the above-identified information and reports share this same knowledge with 10P5 staff. Deliverable 5.7.2 - Accomplish other duties as assigned by 10P5 staff.
