Description
This is not a solicitation announcement. This is a sources sought synopsis only. The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classification (Hubzone, 8(a), Small Business, Small Disadvantaged Business, Veteran Owned Small Business or Service Disabled Veteran Owned Small Business) relative to NAICS 488999 (Size Standard $7 million). Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. Responses to this sources sought synopsis are not considered responses to any potential solicitation announcement. The Department of Veterans Affairs VHA Network 15 Contracting Office is seeking information concerning possible sources to provide transport services for Mobile Magnetic Resonance Imaging (MRI) system for the Dwight David Eisenhower Department of Veterans Affairs Medical Center, 4101 S. 4th Street Leavenworth, KS 66048. The service provider must be accredited by The Joint Commission and must meet manufacturers' performance and technical specifications, ISO standards, Federal Regulations, FDA device certification and other specifications that may apply such as National Electric Code (NEC), Life Safety Code (LSC), and VA Regulations. The VA intends to award a contract to a qualified organization to provide these services for the period of time of 12/01/2013 thru 11/30/2014 base year, with four follow-on option years. A Request for Quotes will be posted on FebBizOpps after the VA concludes its market research to the availability of possible sources. These services are currently being provided by expiring contract VA255-P-0530. The Statement of Work below provides a summary of services required. All interested firms shall provide the following information: company name, address, point of contact, phone number, email address, capability statement, business size and socio-economic classification. Telephone responses will not be accepted. Responses must be received in writing no later than 12:00 p.m. CST October 30, 2013. This notice is to assist the VA in determining sources only. A solicitation is not currently available. If a solicitation is issued it will be announced at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement. Responses to this sources sought is not a request to be added to a prospective bidders list or to receive a copy of the solicitation. Statement of Work 1.1.The contractor shall furnish a Performance Work Statement to include all labor, transportation, and parts necessary to provide prime mover and a qualified licensed driver to pull a Department of Veterans Affairs supplied Mobile Magnetic Resonance Imaging (MRI) system for the VA Eastern Kansas Health Care System Leavenworth, Kansas. 2.Provide a performance work statement for transport services. Contractor shall provide truck/trailer fully capable to transport trailer as listed. The driver shall have a Class A Commercial DRIVERS License (CDL) with appropriate designation to haul the MRI trailer mounted equipment. The estimated total weight with equipment is 63K lbs. with a rear axel wt. of 40K lbs. The driver must possess the ability to transport and setup mobile MRI unit at specified locations for the EKHCS see attached schedule. 3.The drives should possess a valid Cardio/Pulmonary Resuscitation (CPR) certification. 4.The contractor will perform his/her duties before and after normal VA Eastern Kansas Health Care System, Kansas business hours, 7am-5:30pm Monday-Sunday, except Federal Holidays. The VA reserves the right to change shifts as warranted by scan volume. Provider must have experience in performing this task and provide past performance and provide current references of this ability. The required documents shall be provided to the Contracting Officer's Representative (COR) for review. 5.Driver gasoline or diesel powered tractor trailer combination to transport and deliver MRI services in a mobile environment throughout area served. Facilities maintenance of tractor and assists MRI technologists as needed. Drives truck to designated locations per attached schedule, prepares coach, powers up equipment and ensures that stairs, doors, ramps are ready for patient use. The driver shall follow all established safety procedures for set up of the equipment including leveling and initiating the computer software. 6.Driver shall establish connectivity to internet services (IS) applications as well as phone service. The driver shall power down unit which includes preparation of the equipment for transport to and from the designated sites. 7.Driver must inspect truck for defects before to and after trips and submits report indicating truck/coach condition. Must also maintain drivers log according to local and Department of transportation (DOT) I.C. regulations Observes safety and security all times, assists MRI technologist as requested. 8.Work Environment: While performing the duties of this job, the employee is regularly exposed to wet and or humid conditions; moving mechanical parts and outside weather conditions. The employee can be exposed to high, precarious places; fumes or airborne particles; toxic or caustic chemicals; extreme cold; extreme heat; risk of electric shock and vibration. The noise level in the work environment is usually loud. 9.Special Physical Demands: While performing the duties of this job, the employee is regularly required to talk and hear. The employee is frequently required to strand, walk, sit, use hands to finger, handle, or feel; reach with hands and arms; climb or balance and stoop, kneel, crouch, or crawl. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus. 10.Language Skills: Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization. 11.Terms and Conditions: Driver shall transport MRI trailer to the following locations using the tentative MRI schedule for the EKHCS MRI trailer: (See attached schedule) Eastern Kansas Health Care Systems (EKHCS) Locations: a.Munson Army Health Center 550 Pope Ave. Fort Leavenworth, KS 66027, b.Department of Veterans Affairs Medical Center (DVAMC) Kanas City, 4801 E. Linwood Blvd, Kansas City Mo 64128, c.Dwight David Eisenhower (DDE) Department of Veterans Affairs Medical Center (DVAMC), 4101 S. 4TH St. Leavenworth, KS 66048 12. Special Instructions: The contractor shall ONLY respond to service calls placed from COR or designee. The Contracting Officer Representative (COR) for this contract is Carl Campbell, his office phone number is (913) 946-1586 and his email address is Carl.Campbell@va.gov. 13.Identification: The contractor's employee shall wear visible identification at all times while on the premises of the VA Medical Center. Smoking is prohibited inside of buildings. Possession of weapons is prohibited. Enclosed containers, including tool kits, shall be subject to search. Violations of the VA regulations may result in citation answerable in the United States (Federal) District Court. 14.All services for this contract must meet manufacturers' performance and technical specifications, ISO standards, Federal Regulations, FDA device certification and other specifications that may apply such as National Electric Code (NEC), Life Safety Code (LSC), the Joint Commission (JC), and VA Regulations. Also, Contractor must provide the appropriate service documentation at no additional expense. Contractor shall provide hardcopy proof, upon request by COR, that contracting entity is current with all required certifications, licenses and or training. 15. "The C&A requirements do not apply, and that a Security Accreditation Package is not required." The MR Trailer does have VA Network access. This system does link into the VISTA computer network however the contractor shall not have access to the VISTA computer network nor shall the contractor access any PHI and or PII (electronic or hardcopy) at anytime. 16. Contractor shall not perform any service that will result in additional charges without prior approval from the DVAMC Contracting Officer. 17. There is a potential for exposure to blood borne or other infectious material with equipment throughout the hospital. All maintenance persons must use the "Universal Precautions" (i.e. decontamination of medical equipment, wearing protective gloves, aprons, and goggles) during cleaning and maintenance/repair procedures required by this contract. 18.Payment will be made either monthly or quarterly per contracting officer, in arrears, upon receipt of a properly prepared invoice, referencing the period for which payment is due, contract number and/or purchase order number assigned.
