- SF 294 REQUIRED FROM LARGE BUSINESSES Business Management Questionnaire ACH Form Solicitation Document This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.2 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. The solicitation number is CT2149-13 and this solicitation is issued as a Request For Proposals (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005- 70. The North American Industry Classification System code is NAICS 313210 (Broadwoven Fabric Mills). This requirement is unrestricted; small business size standard for this solicitation is 1000 employees. UNICOR, Federal Prison Industries, Inc. intends to enter into a one (1) year base with four (4) option years, firm fixed price requirements type contract. The product is a commercial item: Poly Micro Denier, delivered to one (1) location (WSCS, Waseca, MN). A complete description of the item is in Section B Supplies or Services and Prices/Costs. The estimated quantity for the line item is also shown in Section B. Please see Solicitation Document for complete details. SUBCONTRACTING PLAN: Large Businesses submitting a proposal are required to submit their subcontracting plan, SF 294, as part of their proposal by the scheduled closing date and time. Failure to submit a subcontracting plan may result in the proposal being found technically unacceptable and may not be considered for further evaluation.
