- SCHEDULE QUALITY ASSURANCE PLAN FAR CLAUSES ADDITIONAL PERFORMANCE REQUIREMENTS PERFORMANCE WORK STATEMENT This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the information in FAR Subpart 12.6, using Simplified Acquisition Procedures under the test program for commercial items found at FAR 13.5, as supplemented with the additional information included in this notice. This announcement constitutes the only solicitation; a written solicitation will not be issued. PAPER COPIES OF THIS SOLICITATION WILL NOT BE AVAILABLE. This combined synopsis/solicitation SHALL be posted on FEDBIZOPPS. The RFQ number is N00189-13-T-E083. This solicitation documents and incorporates provisions and clauses in effect through FAC 2005-69 and DFARS Change Notice 20130909. It is the responsibility of the contractor to be familiar with the applicable clauses and provisions. This requirement is unrestricted. The U.S. Navy Fleet Logistics Center Norfolk, VA requests responses from qualified sources capable of providing the rental equipment and services identified in the attached schedule and in accordance with the attached Performance Work Statement via a commercial firm fixed price indefinite delivery requirements contract. Performance will occur at the Norfolk Naval Shipyard, Portsmouth VA from 01 February 2014 through 31 January 2015 (with annual options through 31 January 2019 in accordance with FAR 52.217-9 OPTION TO EXTEND THE TERM OF THE CONTRACT and up to six months of addition option in accordance with FAR 52.217-8 OPTION TO EXTEND SERVICES). Small Business Subcontracting Plan (if quoter is not a small business) Sections of the Request for Quote (RFQ) relating to technical capability information, past performance information, pricing and small business subcontracting shall be submitted as follows: Technical Capability Information: The quoter should provide information that clearly demonstrates its approach to meeting the requirements of the Performance Work Statement (PWS). At a minimum, the contractor shall indicate in writing that their quotation will meet all of the requirements of the PWS. Past Performance Information: The quoter shall demonstrate relevant past performance or affirmatively state they do not possess relevant past performance. Relevant past performance is performance under contracts or efforts since 2008 that is the same as, or similar to, the scope, magnitude and complexity of the work described within this RFQ. In demonstration of such past performance, the quoter shall provide information on no more than three (3) previous contracts whose relevant effort was performed after October 2008. Yet-to-be performed work and work performed prior to October 2008 will not be considered. Quoters may submit performance data regarding current contract performance as long as a minimum of one (1) year of performance has been completed as of the closing date of this RFQ. Quoters shall provide a detailed explanation demonstrating the relevance of the prior contract(s) to the requirements of the RFQ. Past performance information shall be submitted in accordance with the following format: Past Performance Information Form The quoter shall complete a copy of this form for each reference submitted. This form is to be returned along with the quotation. Past Performance Report Form. The quoter should contact each past performance reference and request their completion of this form. Completed forms shall be forwarded electronically by the reference directly to the Contracting Officer (nicole.jacksonheard@navy.mil) on or before the closing date of the RFQ. The Government reserves the right to consider report forms received after the closing of the RFQ. The Government may verify past performance information by contacting some or all of the references provided and may collect additional information through questionnaires, telephone interviews and existing data sources including (but not be limited to) the Contractor Performance Assessment Reporting System (CPARS). The Government reserves the right to obtain information for use in the evaluation of past performance from any and all sources including sources outside of the Government. This information may include contracts other than those identified by the quoter Small Business Subcontracting Plan: All offerors, except small business concerns, shall submit a small business subcontracting plan in accordance with FAR Part 19.7, FAR Clause 52.219-9, and DFARS 252.219-7003. This plan must be approved by the Contracting Officer and will be incorporated into the contract upon award. The offeror shall provide goals, expressed as dollars and percentages of the total subcontract value. The goals may provide for participation by a prime contractor, joint venture partner, teaming arrangement member or subcontractor. Subcontracting goals expressed in the plan will be incorporated into, and become part of, any resultant contract. Note: The Government's subcontracting goal is 5%. Price: The quoter shall submit pricing in accordance with the Schedule of Services contained within the RFQ. There shall be no reference to pricing within the Technical or Past Performance information. The completion and submission of the above items will constitute a quote and will be considered the vendor's unconditional assent to the terms and conditions of this solicitation and any attachments and/or exhibits. Failure to provide all of these documents by the required date and time may preclude a quotation from award consideration at the discretion of the Government. Quotations are valid for 60 days. 52.212-2 EVALUATION - COMMERCIAL ITEMS (JAN 1999) ADDENDUM The Government will award a contract resulting from this Request for Quotation (RFQ) to the responsible quoter whose quotation conforms to the RFQ and will be the most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate quotations: 1. Technical Capability; 2. Past Performance; 3. Price 4. Small Business Subcontracting Plan (for offerors other than small businesses). Contractors who fail to possess acceptable technical capability will not be considered for award and the other elements of their quotation will not be evaluated. For purposes of award among all of the technically acceptable quotations, past performance is considered to be more important than price. Quoters (other than small businesses) must also have an acceptable small business plan in order to be awarded a contract. 1. Technical Capability: The Government shall evaluate the information provided using the following ratings and definitions: RATING DEFINITION Acceptable Quoter has the technical capability to perform the requirements of the RFQ Unacceptable Quoter does not have the technical capability to perform the requirements of the RFQ 2. Past Performance: The Government will assess the degree of confidence it has in each quoter's ability to perform the services described in the Performance Work Statement based on the quoter's demonstrated record of past performance. That degree of confidence will be assessed based on the relevance of the quoter's past performance and the quality of that past performance. Relevance is determined by the past performance's similarity to the current requirement based upon its scope, magnitude and complexity. Scope is experience in the areas specified in the Performance Work Statement; Magnitude is the dollar value of the work actually performed during the period since Aug 2008. Complexity is the required coordination, technical and managerial intricacy of effort. Quoters who lack relevant past performance will receive a Neutral rating and will not be evaluated favorably, or unfavorably, for past performance Relevance will be assessed using the following ratings: RATING DEFINITION Very Relevant Present/past performance effort involved essentially the same scope and magnitude of effort and complexities this solicitation requires. Relevant Present/past performance effort involved similar scope and magnitude of effort and complexities this solicitation requires. Somewhat Relevant Present/past performance effort involved some of the scope and magnitude of effort and complexities this solicitation requires. Not Relevant Present/past performance effort involved little or none of the scope and magnitude of effort and complexities this solicitation requires. Past Performance Confidence Assessment Rating - The overall assigned rating for Past Performance will be the Past Performance Confidence Assessment rating. The assignment of this rating will be based on the quality of relevant past performance and will also consider the relevance of the information, source of the information, context of the data and general trends in the contractor's performance. The quality of performance under a past performance reference that has no relevancy to this solicitation's requirement will not be considered in the overall assessment of Past Performance Confidence. This consideration is separate and distinct from the Contracting Officer's responsibility determination. The assessment of the quoter's past performance will be used as a means of evaluating the relative capability of the quoter and other competitors to successfully meet the requirements of this solicitation. In determining the rating for past performance, the Government will give greater consideration to the contracts which the Government feels are most relevant to the services required by this solicitation. Past performance confidence will be assessed using the following ratings: RATING DEFINITION Substantial Confidence Based on the quoter's recent/relevant performance record, the Government has a high expectation that the quoter will successfully perform the required effort. Satisfactory Confidence Based on the quoter's recent/relevant performance record, the Government has a reasonable expectation that the quoter will successfully perform the required effort. Limited Confidence Based on the quoter's recent/relevant performance record, the Government has a low expectation that the quoter will successfully perform the required effort. No Confidence Based on the quoter's recent/relevant performance record, the Government has no expectation that the quoter will be able to successfully perform the required effort. Unknown Confidence (Neutral) No recent/relevant performance record is available, or the quoter's performance record is so sparse that no meaningful confidence assessment rating can be reasonably assigned. 3. Price: The Government will multiply the estimated quantities provided in the Schedule by the unit pricing proposed by the quoter to determine the estimated amount for each line item. The estimated amount for each line item will then be added together to determine the total estimated price which will become the evaluated price. The Government will include FAR 52.217-8 OPTION TO EXTEND SERVICES in its price evaluation Prices will be evaluated for reasonableness in accordance with FAR 13.106-3. Contractors responding to this RFQ are advised that prior to award, the Government may request submission of additional information or data to support price reasonableness. Failure to submit the requested information may result in disqualification of a quotation. 4. Small Business Subcontracting Plan (for offerors other than small businesses): RATING DESCRIPTION ACCEPTABLE Plan meets proposal requirements and is in compliance with FAR Part 19.7, FAR Clause 52.219-9, and DFARS 252.219-7003. The plan provided goals, expressed as dollars and percentages of the total subcontract value. UNACCEPTABLE Plan does not meet proposal requirements and is NOT in compliance with FAR Part 19.7, FAR Clause 52.219-9, and DFARS 252.219-7003.
