- CLARIFICATION: ALL SUBMITTALS ARE DUE BY 10:00 PM (LOCAL) THURSDAY, SEPTEMBER 26, 2013. This is a market survey for the removal and installation of modular tact-tile carpet in two (2) buildings at the Mike Monroney Aeronautical Center, Oklahoma City, OK. The responses to this market survey will be used for informational purposes only. This is not a screening information request or request for proposal of any kind. The FAA is not seeking or accepting proposals at this time. The FAA will not pay for any information received or costs incurred in preparing the response to the market survey. Therefore any cost associated with the market survey submission is solely at the interested vendor's expense. The purpose of this market survey is to solicit statements of interest and capabilities from small businesses, service-disabled veteran-owned small businesses, and 8(a) certified firms, capable of providing the repair and refurbishment of the crane, per the Scope of Work. This market survey is also being conducted in order to obtain the information necessary to determine whether adequate competition exists to set-aside the competition among small businesses, service-disabled veteran-owned small businesses or 8(a) certified firms. Interested firms are required to submit the attached Business Declaration form and a copy of their SBA 8(a) certification letter, if applicable. Draft SCOPE OF WORK: Provide all labor, equipment and materials required as necessary for a the removal of approximately 35,026 SF of existing modular carpet and installation of Interface Architectural Line "To Scale 7772 Blueprint" Modular Carpet, installed with TacTile OR EQUAL. Approximately 4,820 LF of rubber base will be replaced. Work will include moving, lifting, relocation of furniture, desks, podiums, and other normal pieces in a classroom or office setting as required to perform the installation. Work will occur at off shift hours, and/or by arrangement for special circumstances. Existing carpet & base will be removed from project location and recycled by contractor. In order to make this determination the FAA requires the following from interested vendors: 1. Capability Statement - This document should identify: Type of services provided by your firm, Size and type of services provided by previous contracts (elaborate and provide detailed information and past performance), Number of years in business 2. Specify the brand name and style of carpet you would provide and complete the attached Buy American Act certification. Respondents must be active, or in the re-activation process in the System for Award Management (SAM) (www.sam.gov). Please respond by providing the requested information, to include a filled-out and signed copy of the Business Declaration form and Buy American Act certification.
