- This is a combined solicitation/synopsis for commercial services, prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes only solicitation, quotations are being requested and a written solicitation will not be issued. The solicitation number, N00259-13-T-0406 is issued as a request for quotation (RFQ). The North American Industry Classification System (NAICS) Code for this acquisition is 515210; Size: 15.0M. All interested bidders shall submit quotations electronically by email to reynaldo.devera@med.navy.mil or by facsimile at 619-532-5596, attention Rey de Vera. E-mail submissions are limited to 2MB. The submitter should confirm receipt of facsimile and email submissions. Quotations are due on or before 25 September 2013 at 04:00 PM Pacific Standard Time to be considered responsive. CLIN 0001 CABLE TV SERVICE QTY 12 Month $________ To be provided in accordance with the Statement of Work Period of Performance: 01 October 2013 to 30 September 2014 Contractor must be registered to the System for Award Management (SAM) prior to award. AVAILABILITY OF FISCAL YEAR 2014 FUNDS. THE GOVERNMENT OBLIGATION MADE HEREUNDER IS CONTINGENT UPON THE ENACTMENT OF THE FISCAL YEAR 2014 APPROPRIATION ACT. THIS ORDER IS ISSUED SUBJECT TO THE 2014 DOD APPROPRIATION ACT OR AN EXTENDED FISCAL YEAR 2013 CONTINUING RESOLUTION ACT AND IS SUBJECT TO ALL PROVISIONS OF WHICHEVER ACT BECOMES APPLICABLE. THE CLAUSE 52.232-18 REFERENCED HEREIN APPLIES TO THIS CONTRACT. STATEMENT OF WORK Contractor shall install basic business package with local channel capabilities in standard definition television (SDTV) to the 55 locations identified in the physical locations. If required, contractor shall also provide necessary parts for a functional system to include but is not limited to cables, connectors, amplifiers, signal boosters, and face plates. Contractor shall, maintain, troubleshoot, align, and repair all parts of the hospital's (Bldg 937 & 926) TV system to include all parts of the system but is not limited to, cables, connectors, amplifiers, and signal boosters. Contractor shall perform unscheduled and emergency maintenance in a timely fashion on all equipment items where an equipment deficiency exists. Contractor shall install cable to room 2270 which is listed in the rooms identified below. The room requires cable drop. Install faceplate and connect cable to faceplate and connect to hospital cable television system. Period of Performance: Oct 1, 2013 - Sep 30, 2014 Physical Location: Naval Hospital Lemoore 937 Franklin Ave NAS Lemoore, CA 93246 Bldg 937: Rooms: 1100, 1128, 1138, 1140, 1144, 1154, 1266, 1271, 1272, 1280, 1286, 1300, 2101, 2200, 2270, 2315(2), 2317, 2460(2), 2456(2), 2448(2), 2446, 2444, 2443, 2304, 2306, 2308, 2310, 2312, 2314, 2101, 2135, 2838, 2224, 2300, and outlets in Atriums/Patient waiting areas throughout facility (16 patient waiting areas). Bldg 926: Patient waiting lounge only. Supplies and Equipment: Any supplies or equipment needed for installation and maintenance will be supplied by the vendor.
