Description
The Ralph H. Johnson VA Medical Center located at 109 Bee Street in Charleston, South Carolina is seeking a contractor to provide transitional housing community based health care for homeless veterans. This is a firm fixed price contract. Contract period shall approximately commence March 1, 2014 through February 28, 2015 with four (4) option years. The Contractor shall furnish services to beneficiaries from whom such care is specifically authorized by the RHJ VA Medical Center and who are homeless and many with substance abuse and/or other mental health diagnosis. The types of patients to be cared for under this contract will normally require care and services over and above the level of room and board. The contractor shall provide each authorized veteran the following basic services: a.room and board, including 3 nutritionally adequate meals a day, 7 days a week and availability of nutritious snacks between meals and bedtime for those requiring or desiring additional food, when it is not medically contraindicated. b.Clean and sanitary housing shall be provided to all veterans. The housing shall include appropriate space for clothing and personal items c.Indoor recreation/lounging areas shall be provided for all Veterans. d.Transportation to all meetings/appointments/interviews in the community or at the VAMC The facility must be connected to the Homeless Management Information System (HMIS) at the facility's expense and facility must be sprinkled prior to award of contract. The facility must be located within the Tri-County Area (Berkley, Dorchester or Charleston County).
