CONTINUATION PAGE QUESTIONS AND ANSWERS 1. Is Liability Insurance coverage for the State of Nevada? a.No, Liability Insurance coverage is for the State of Texas. 1-Will the 2 VA's in Texas simply contact the service provider and schedule a procedure whenever they have a patient(estimated 10 patients per month for each of the 2 sites) requiring a lithotripsy procedure? Or, would they rather schedule certain days each month for our mobile Lithotripsy system to setup in each of the facilities to handle the patients scheduled? If they want to just contact us when needed, we would need to dedicate a system to VA Texas to handle any patients on any day which is fine for us as we can just provide a monthly rate for system and techs. If they agree to schedule certain days we can utilize the system at other sites on all other days not scheduled by VA Texas. 2-How many days per month would they require? We can bid either way, just need to know. a.Here at CTVHCS because of what the service is, our procedures are set up for 4 to 6 (no less than 2) patients on a scheduled day at least twice a month. Most of the time the procedures are scheduled at a certain time of the month from month to month. "We schedule the patient (sometimes months in advance but patients cancel or physicians have to reschedule) "We contact the vendor with proposed dates then confirm 48 to 72 hours in advance "This works best because the vendor allows us 24 hours to cancel.
