Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is W91YU0-13-T-0187, 0010399209 and is issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-69 The associated North American Industrial Classification System (NAICS) code for this procurement is 541990 (All other Professional Scientific and Technical Service) Size standard is $6.0 This RFQ is being issued as Firm Fixed Price, 100% SMALL BUSINESS SET ASIDE All responsible sources may submit a quotation which shall be considered by the agency These items/supplies are for use at William Beaumont Army Medical Center, (WBAMC), El Paso, TX All responsible Contractors shall provide an offer for the following. CLIN 0001: The Contractor shall provide a non-personal service for Onsite Maintenance and Repair Services in compliance with the Manufacturer's specifications for the medical surgical instruments and containers at William Beaumont Army Medical Center (WBAMC). Services to be provided such as sharpening, repairs, etching and surgical instrument container repairs for the various medical surgical instruments, for the department of CMS located at William Beaumont Army Medical Center, (WBAMC), El Paso, TX, in accordance with the attached Statement of Work (SOW). The contractor shall provide all resources, labor, travel, parts, equipment and management necessary to keep the medical instruments and containers in standard operating condition. Period of Performance: 1 September 2013 through 31 Jan 2014 EVALUATION OF FACTORS: Award will be made based on Technical Acceptable Only. a.Technicians MUST be trained and certified to perform necessary service on all medical equipment, such as (Sharpening, repairs, etching and surgical instrument container repairs) b.Technicians MUST be must be skilled in the repair and maintenance to the instruments and containers in a serviceable condition in accordance with the manufacturer's specifications, and continuous product information updates. c.Technicians MUST ensure medical equipment is 100% operational. Equipment shall be restored to a serviceable condition within twelve hours after arrival at the Facility or on-site repair visit. d. Technicians MUST ensure uptime of 98% (number of instruments available for use/number of instruments available for service). Technical Factors are more important than Price. Price must be determined to be fair and reasonable. Contractor shall submit specification with the quote.
