- Description
- The Government to negotiate on a sole source basis, under simplified acquisition procedures, with D. D. Bainbridge & Associates Inc., 595 Lyndon Center Road, Cuba, New York 14727, for their quality calendar services to include data entry and ongoing reporting and tracking services. These services are used by rural critical access hospitals and this service has not been found to be available from any other sources. These services are critical to maintaining the accreditation of the hospital and provide reporting and tracking features that the hospital cannot provide for itself. These services were previously contracted by the State of Arizona for the Hopi Health Care Center in Polacca, Arizona to D. D. Bainbridge & Associates but the State of Arizona can no longer provide this service. Since it's no longer being provided by the State of Arizona the Phoenix Area Indian Health Center intends to enter into a firm fixed-price contract with D. D. Bainbridge & Associates with a Base Period of 12 months and four 12-month options. This is not a request for competitive quotes. If you believe your firm is able to provide the service required above, please provide a capabilities statement of how your firm will provide the required services to meet the needs of the Government as specified above. All responses to this notice must be received by 4pm Arizona Standard Time on September 9, 2013.
