THIS IS A SOURCES SOUGHT NOTICE REPRESENTING A MARKET SURVEY AND IS NOT A REQUEST FOR PROPOSALS, PROPOSAL ABSTRACTS, QUOTATIONS OR INVITATION FOR BIDS The Department of Veteran Affairs, NCA 8, James A. Haley Hospital, Tampa,Florida is seeking responsible sources to provide service as outlined in the Statement of Work. This is market research for informational and planning purposes only. This is not a solicitation and does not obligate the government to issue a solicitation. Please be advised that the Government will not pay for any information submitted under this announcement. Your response should identify your company name, point of contact with contact information, indicate all areas you are capable of providing service, capabilities statement and identify your business size, and if you are any of the following: Federal Supply Holders, 8(a) Business, Small Business, Veteran-Owned Small Business, Service-Disabled Veteran- Owned Small Business, HUB Zone Small Business, Small Disadvantaged Business, and Women- Owned Small Business concerns may respond by submitting a qualifications letter stating the following information: 1. Capability Statement that exhibits clear and convincing evidence that your firm has the capabilities and experiences to perform the listed tasks or deliver the required items as stated in the below Statement of Work. 2. Telephone responses will not be accepted. Responses must be received in writing no later than Noon EST August 23, 2013. Responses should clearly state the ability to provide the service. If a solicitation is issued, it will be announced at a later date, and all interested parties must respond to that solicitation announcement separately. Statement of Work (SOW) Domiciliary Food Service Delivery Place of Performance: James A. Haley Veterans Administration Medical Center (JAHVAMC) 13000 Bruce B. Downs Blvd Tampa FL & the Domiciliary located at 4700 East Habana Ave, Tampa FL Section 1.0 Description of Services/General Information 1.1 SCOPE OF WORK: 1.1.1 The contractor shall provide all personnel, supervision, and services necessary to perform Domiciliary Food Service delivery services as defined in this SOW. The contractor shall provide a 16' refrigerated truck with lift gate w/cart stop and tie down equipment or equivalent. Deliveries shall be made consisting of six racks/dollies containing cold food for lunch, dinner and next day breakfast and any miscellaneous dry operation supplies. Delivery includes pick up and return of prior days empty food racks/dollies and pans. 1.2 HOURS OF OPERATION: 1.2.1. The contractor shall perform food delivery service 3 days per week (Tuesday, Thursday, and Saturday) including all federal and other holidays. Food shall be picked up at 8:30 AM at JAHVAMC and delivered at 9:00 AM to the Domiciliary at 4700 East Habana Ave, Tampa Fl. Approximately 20 miles round trip. 1.3 SPECIFIC TASKS: 1.3.1. Driver shall report with the truck, refrigerated on and ready to begin transporting prepared food product from Nutrition & Food Service located off of 131st Ave Tampa Fl. Six food racks/dollies will be picked up at the truck loading dock area at JAHVAMC kitchen area. 1.3.2. Certify load contents. Seal truck, Leave JAHVAMC Tampa loading dock area at 8:30am. 1.3.3. Deliver prepared meals to the Domiciliary program Building at 4700 East Habana AVE. Tampa, Fl at 9:00 AM. Off-load food items for domiciliary program in parking lot and take to 6th floor of the domiciliary kitchen area. Obtain signed invoice. 1.3.5. Load all previous day's food racks and pans. Certify load contents. Seal truck. Leave domiciliary program Building at 4700 East Habana AVE. Tampa, Fl. Off-load at JAHVAMC loading dock and transport to JAHVAMC kitchen location. Submit invoices to receiver. 1.3.6. Refrigerated contents are to remain at 35 to 40 degree F at all times. 1.4 Qualifications: 1.4.1. The driver shall posses at a minimum, a State of Florida Class B safe Commercial Drivers license. 1.4.2. The driver shall present a neat and clean appearance to include the uniform (if applicable) and general clothing in good repair. 1.4.3. Drivers shall have evidence of Hazard Analysis Critical Control Point (HACCP) training, i.e. food handling/transport procedures. (The Veterans Administration (VA) will provide orientation if scheduled one week in advance.) 1.5 General Information: 1.5.1. The contractor shall be responsible for the truck and driver as well as provide liability insurance for the contents being transported. 1.5.2. The contractor shall provide coverage for sick leave and other absences. 1.5.3. Truck is to be swept and cleaned between each delivery. Spills are to be reported to receiving site and cleaned before next load. The truck shall be sanitize regularly and be free from all transport debris. 1.5.4. Truck seal shall remain intact until broken by VA receivers at domiciliary location and JAHVAMC. 1.5.6. Any delays in transport arrival times should be communicated by company to receiving site. 1.5.7. Truck shall be equipped with straps for securing mobile food carts which will contain the prepared meals. 1.5.8. Company must provide records of driver's safety training, certifications and licenses as required by law 1.5.9. Contractor will be trained as required by VA regulations. Section 2.0 Government Furnished Property (GFP) and Services: 2.1. VA employees will support the delivery truck driver with loading and unloading of truck and securing carts.
