Description
THIS IS A SOURCES SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR PROPOSAL OR QUOTE; THE VA IS NOT SEEKING PRICING OR OFFERS. The Department of Veterans Affairs, VISN 20, in support of VA Portland Medical Center, is performing market research to compile data regarding: (1) The availability and capability of qualified small business concerns; (2) Identify if there are sufficient number of qualified Service-Disabled Veteran-Owned Small Business, Veteran Owned Small Business, HUB Zone 8(a), Women-Owned Small Business, or Small Disadvantaged Business, Small Business concerns relative to the subject North American Industry Classification System (NAICS) code. Responses to the information requested will assist the Government in determining the appropriate procurement method, including any set-aside decision. Respondents should clearly describe their capabilities to include management experience, equipment and personnel or the ability to obtain them. Furthermore, interested sources must also meet all requirements of Federal, State, and Local Municipal codes regarding operations of this type service. The NAICS code assigned is: 562211 Hazardous Waste Treatment & Disposal at the small business size standard: $35.5M. Not responding to this Sources Sought does not preclude participation in any future or potential solicitation, or Request for Quotation. It is the intent of VA Portland Medical Center to use the information gathered for market research purposes only. DESCRIPTION OF THE REQUIREMENT: The Contractor shall furnish all personnel, equipment, insurance, proper licensing, tools, materials, travel, transportation, and supervision necessary to provide Radioactive Waste Packaging and Transport at the VA Portland Medical Center, 3710 SW U.S. Veterans Hospital Road, Portland, OR 97239. The contractor shall have and maintain the appropriate state and Federal license(s) required to perform in accordance with (IAW) the attached Statement of Work. The projected performance period consists of: (Anticipated start date is 9 Sept 2013 thru 13 Sept 2013. These dates may be adjusted based upon actual date of award. POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE: 1. Company name, address, phone number, primary contact(s), e-mail address, NAICS code(s), business size i.e. SDVOSB, VOSB, 8A, etc., and DUNS Number. Proof of Central Contractor Registration and Vet Biz registry (if applicable) is highly desirable. Identify all Joint Ventures and other Teaming arrangements. 2. A brief capability statement demonstrating past performance with relevant contract services in Prime Contractor role. 3. Disclaimer: This notice does not obligate the Government to award a contract or otherwise pay for the information provided in response. The Government reserves the right to use information provided by respondents for any purpose deemed necessary and legally appropriate. 4. Submit Capability Statements to: Department of Veterans Affairs, NCO 20 Attn: Ross Byrne, no later than 15:00 PM PST, 16 Aug 2013. Responses must be submitted via e-mail to: Ross.Byrne@va.gov Note: Do not include Proprietary, classified, confidential, or sensitive information in responses. STATEMENT OF WORK - DRAFT 1. Contract Title. Radioactive Waste Packaging & Transport 2. Background. The Research department generates long-lived radioactive waste such as Sodium-22, Chlorine-36, and Tritium. This waste has a long half-life and must be shipped for burial to a designated radioactive waste site through a licensed broker. Long-lived waste is sent to a secure waste facility because the waste can take up to 120 years to decay. Portland VA already has a site permit for radioactive waste storage with US Ecology. A licensed transport Contractor is required to package, receive, transport, and deliver the waste to US Ecology in Washington 3. Scope. The Contractor shall possess a license to package, receive, document, transport, and deliver research radioactive waste in accordance to State of Washington, State of Oregon, city, county, Department of Transportation, and US Nuclear Regulatory Commission regulations. 4. Specific Tasks. The Contractor shall ensure the security of the radioactive waste, follow all regulations in regards to the receipt, transport, and delivery of the radioactive waste, keep accurate inventories of the waste they are transporting, keep accurate transport records detailing the date of transport, class of waste, isotope, activity, and contact information. 5. Performance Monitoring The Contractor shall have a license to receive and transport radioactive waste. By having this license, the Contractor is regularly inspected both by State and Federal agencies. Any major violations would be sent out for public notice. The VA Radiation Safety Officer will receive notice of these violations through list serve communications. 6. Security Requirements The Contractor shall secure the radioactive waste that they receive and transport to ensure that unauthorized access, loss, or theft does not occur. This is a regulatory requirement that they are well aware of. The contractor employees shall not have access to VA sensitive or computer information and will not require routine access to VA Facilities. The contractor employees shall require intermittent access only and will be escorted by VA employees while at VA Facilities. No background investigation is required 7. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). NA 8. Other Pertinent Information or Special Considerations. NA a. Identification of Possible Follow-on Work. NA b. Identification of Potential Conflicts of Interest (COI). NA c. Identification of Non-Disclosure Requirements. NA d. Packaging, Packing and Shipping Instructions. Radioactive waste must be package in 55 gallon drums classified for transporting radioactive waste. Radioactive waste must be kept secure during receipt, transport, and delivery. All applicable regulations must be followed. e. Inspection and Acceptance Criteria. The Contractor shall be already inspected for regulatory compliance by various State and Federal agencies. The Radiation Safety Officer will be made aware of any major violations of regulations made by the company. 9. Risk Control NA 10. Place of Performance. The Contractor shall package and receive the radioactive waste onsite at the Portland VA Medical Center. The Contractor shall then transport the waste to US Ecology's disposal site located in Richland, Washington in Benton County. 11. Period of Performance. 9 Sept 2013 thru 13 Sept 2013
