- INTRODUCTION: THIS IS A NOTICE OF INTENT, NOT A REQUEST FOR A PROPOSAL. A SOLICITATION DOCUMENT WILL NOT BE ISSUED AND PROPOSALS WILL NOT BE REQUESTED. The National Heart, Lung, and Blood Institute (NHLBI) Office of Acquisition (OA) intends to negotiate and award a purchase order on a noncompetitive sole source basis to International Orthodox Christian Charities (IOCC) Podoconiosis Research, Addis Ababa, Ethiopia to conduct a research trial that compares a standardized parent-directed health behavior change intervention (SHE) promotes consistent shoe wearing by an index child aged 3 to 6 regarding footwear practices among children at high risk for podoconiosis in Ethiopia. To all produce intervention materials for a podoconiosis prevention and education research study. This is a Follow-On requirement. Background : The National Institute of Health (NIH) is the nation's leading medical research agency and the primary Federal agency conducting and supporting medical discoveries that improve people's health and save lives. The National Human Genome Research Institute (NHGRI), Social & Behavioral Research Branch (SDRB) in partnership with the Mossy Foot Treatment and Prevention Association (MFTPA) in Ethiopia, has conducted formative research to support the development and delivery of prevention and education intervention to address the problem of podoconiosis in the Wolaita region of Ethiopia. The National Human Genome Research Institute has been partnering with the Mossy Foot Treatment and Prevention Association (MFTPA) and Addis Ababa University in conducting extensive qualitative research regarding footwear practices among children at high risk for podoconiosis in the Wolaita zone of Ethiopia. A complement of focus groups, individual interviews and case studies with parents and children now have been completed in four target communities. The intervention was developed and testing started in September 2012 with necessary approvals gained from NHGRI's IRB and the government of Ethiopia. This has allowed us to start collecting preliminary data to base our study on. Over the past 8 months the contract workers in Ethiopia have determined appropriate candidates and met with families in the Woliata Zone of Ethiopia. After reviewing the initial data, it was determined that it is vital to this project that the next step in data collection be undertaken to utilize the effects of the rainy season in this zone. Purpose and Objective: The overarching objectives of the activities proposed is to conduct a research trial that compares a standardized parent-directed health behavior change intervention (SHE) promotes consistent shoe wearing by an index child aged 3 to 6 (as reported by the participating adult in affected families who receive shoes from the MFTPA - henceforth "high risk children"). The contractor will continue to oversee the conduct of the 3 group controlled intervention trial conducted with families in rural Ethiopia. The activities for the coming year include two rounds of data collection, one timed to occur 10 months after the intervention and the second 5 months later, in 1800 affected and unaffected households. Survey interviewers will return to complete surveys with participating parents to assess whether targeted children are wearing shoes. Additionally, follow-up surveys will reassess parent's beliefs about the causes of podoconiosis, confidence to overcome barriers to their child's wearing shoes, encouragement of shoe wearing and foot hygiene, and stigma. Anticipated Period of Performance : September 17, 2013 through September 16, 2014 Option Year Period One: September 17, 2014 through September 16, 2015 Contractor Requirements: The contractor shall provide scientific oversight of the conduct of the research trial. The specific requirements are to: • Possess extensive expertise in the epidemiology of podoconiosis and experience in conducting public health interventions in the Wolaita zone of Ethiopia; • Speak Amharic and Wolatinya (dialects used in the Wolaita zone); • Relationships with community and religious leaders; • Experience conducting surveys with those speaking Wolatinya; and • Lead and steer the data collection activities that will take place in Wolaita, Ethiopia, in collaboration with the local communities and the Mossy Foot Treatment and Prevention Association. Justification : Podoconiosis is a neglected tropical disease localized predominantly in regions of sub-Saharan Africa including Ethiopia. IOCC Researcher in Global Health at the Brighton & Sussex Medical University, UK, is one of the world leading experts on podoconiosis. IOCC possesses expertise in the epidemiology of podoconiosis and experience in conducting qualitative and quantitative research involving podoconiosis in the Wolaita region of Ethiopia, and having lived and worked in the area for over a decade. Through research with IOCC several genes have been identified that have been associated with higher risk of the disease provided susceptible individuals are exposed to clay soil that causes podoconiosis. Researchers have a long-standing relationship with the Mossy Foot Treatment and Prevention Association, a local Non-Governmental Organization that provides services to patients through 15 health clinics, which are the clinical sites for the proposed research activities. Regulatory Authority : This acquisition is conducted under the authority of the Federal Acquisition Regulations (FAR) Subpart 13.106-1(b) Soliciting from a single source, only one responsible source and no other supplies or services will satisfy agency requirements. Additional Information : Industry Classification (NAICS) Code is 541712, Research and Development in the Physical, Engineering, and Life Sciences (except Biotechnology) with size standard of 500 Employees. The acquisition is being conducted under FAR Part 13, simplified acquisition procedures, therefore the requirements of FAR Part 6 B Competitive Requirements are not applicable (FAR Part 6.001). The resultant Contract will include all applicable provisions and clauses in effect through the Federal Acquisition Circular (FAC) 05-69 (August 1, 2013). This requirement is under the SAT of $150,000.00. This synopsis is not a request for competitive proposals. However, interested parties may identify their interest and capability to respond to this notice. Responses to this notice shall contain sufficient information to establish the interested parties' bona-fide capabilities for fulfilling the requirement and include: unit price, list price, shipping and handling costs, the delivery period after contract award, the prompt payment discount terms, the F.O.B. Point (Destination or Origin), the Dun & Bradstreet Number (DUNS), the Taxpayer Identification Number (TIN), and the certification of business size. All offerors must have an active registration in the System for Award Management (SAM) www.sam.gov." A determination by the Government not to compete this proposed contract based upon responses to this notice is solely within the discretion of the Government. The information received will normally be considered solely for the purposes of determining whether to proceed on a non-competitive basis or to conduct a competitive procurement.
