RFQ# HSCG23-13-Q-PBJ004 Description: Emergency Medical Technician Training. This acquisition is being issued in accordance with FAR Subpart 13. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with the additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The Request for Quote number is HSCG23-13-Q-PBJ004. The North American Industry Classification System (NAICS) Code for this requirement is 611699 with a size standard of $7.0 Million. This requirement is a TOTAL SMALL BUSINESS SET ASIDE; submissions that do not meet the small business requirement will not be considered. This solicitation documents and incorporates provisions and clauses that are in effect through FAC 2005-66 (February 28, 2013). It is the responsibility of the contractor to be familiar with the applicable clauses and provisions. The U.S. Coast Guard intends to establish a single award Purchase Order for the services detailed in the Performance Work Statement (PWS). FAR 52.212-1 Instructions to Offerors - Commercial items (Feb 2012) and is supplemented as follows: Offerors shall submit an electronic version of their quote via up loading to FBO NLT August 22, 2013 at 1400 ET and shall contain the following information: Solicitation Number; Time Specified for receipt of offers; Name; Address; Telephone Number of Offeror; Terms of the expressed warranty; Price, Payment terms, Any discount terms, CAGE code, DUNS number, Tax ID number and acknowledgement of all solicitation amendments. Offerors shall submit their Firm Fixed Price quote per course that is inclusive of all tuitions and fees. (End of clause) NOTICE FOR FILING AGENCY PROTESTS It is the policy of the United States Coast Guard (USCG) to issue solicitations and make contract awards in a fair and timely manner. The Ombudsman Program for Agency Protests (OPAP) was established to investigate agency protest issues and resolve them without expensive and time-consuming litigation. OPAP is an independent reviewing authority that is empowered to grant a prevailing protester essentially the same relief as the Government Accountability Office (GAO). Interested parties are encouraged to seek resolution of their concerns within the USCG as an Alternative Dispute Resolution (ADR) forum, rather than filing a protest with the GAO or some external forum. Interested parties may seek resolution of their concerns informally or opt to file a formal agency protest with the contracting officer or Ombudsman. Informal forum with the Ombudsman. Interested parties who believe a specific USCG procurement is unfair or otherwise defective should first direct their concerns to the cognizant contracting officer. If the contracting officer is unable to satisfy the concerns, the interested party is encouraged to contact the USCG Ombudsman for Agency Protests. Under this informal process, the agency is not required to suspend contract award performance. Use of an informal forum does not suspend any time requirement for filing a protest with the agency or other forum. Formal Agency Protest with the Ombudsman. The purpose of this Performance Work Statement (PWS) is to obtain contractor support to provide Emergency Medical Technician (EMT) training in order to meet the demands of the field for additional EMTs. Specifically, this PWS requires the contractor to provide all management, supervision, expertise, personnel, materials and equipment necessary to provide EMT Training in accordance with the National Highway Traffic Safety Administration (NHTSA) National EMS Educational Standards and NHTSA Emergency Medical Technician Instructional Guidelines. Additionally this course will provide E-3 through E-9 personnel of any rating the Emergency Medical Technician skills necessary to quickly assess, stabilize, and transport a victim of a sudden illness or injury. Upon completion, an EMT will know their roles and responsibilities as part of the Coast Guard's emergency medical services (EMS) system and employ all means within the scope of their training to deliver a stable patient, whenever possible, to a higher level of EMS care. 1.2 BACKGROUND. 1.2.1 Mission: The mission of the Coast Guard EMT training is to prepare enlisted personnel whose duties include the emergency assessment and transport of the sick and injured. Trainees typically include, but are not limited to, Health Services Technicians, Aviation Survival Technicians, and members of other ratings involved in operational missions likely to encounter victims of severe trauma or illness. 1.2.2 Problem Statement. Over the years, the National Registry of Emergency Medical Technicians has maintained four general levels of care provided to the public. Each level of care has a specific Scope of Practice that being "A description of what a licensed individual legally can, and cannot, do." The 2010 National EMS Scope of Practice Model was established by the National Highway & Traffic Safety Administration (NHTSA) as a vision for the future of EMS in the United States. Higher, more efficient quality out-of-hospital care is a key factor and an important part of the United States health care system. Each level of care has been evaluated and each level has been increased on both the knowledge and skill aspect to meet the demands of our customers. Many patients have complicated traumatic or medical conditions that require considerable knowledge, skill, and judgment to be treated effectively in the out-of-hospital setting. The current and future provider levels by the NREMT are as follows: Beginning fy12: New Levels as of 1 Jan: First Responder NR-Emergency Medical Responder EMT-Basic NR-Emergency Medical Technician EMT-Intermediate (I-85 and I-99) NR-Advanced Emergency Medical Technician EMT-Paramedic NR-Paramedic At the onset of fy12, the USCG EMS School educated at the EMT-Basic level. Beginning January 1, 2012, the NREMT no longer offered cognitive or psychomotor skills testing on those levels. It was expected that the Coast Guard would certify our personnel to the EMR level however, after a complete and thorough review of care provided and the austere environment in which we are called to operate it was determined Emergency Technician was the correct CG certification level. As a result, the CG EMS School to change course mid-year increasing our course length from 3 - 7 weeks causing throughput to drop nearly half. 1.3 CONTRACTOR PERSONNEL. 1.3.1 Qualified Personnel. The Contractor shall provide qualified personnel, with minimum certifications as a NREMT; it is preferred that the instructors have further qualifications to include nurse practitioners, physician assistants, and/or paramedics. The contract company must be familiar with providing EMS care in the austere environments the Coast Guard routinely operates in i.e. high seas. Contractor shall maintain a minimum of 6 to 1 instructor to student ratio for all practical exercises. 1.3.2 Class Manager. The Class Manager shall be a single point of contact for the Contracting Officer (KO) and the Contracting Officer's Representative (COR). It is anticipated that the Class Manager shall be one of the senior level employees provided by the Contractor for this work effort. The name of the Class Manager, and the name(s) of any alternate(s) who shall act for the Contractor in the absence of the Class Manager, shall be provided to the Government as part of the Contractor's proposal. During any absence of the Class Manager, only one alternate shall have full authority to act for the Contractor on all matters relating to work performed under this PWS. The Class Manager and all designated alternates shall be able to read, write, speak and understand English. Additionally, the Contractor shall not replace the Class Manager without prior consent from the Contracting Officer. 1.3.2.1 The Class Manager shall be available to the COR via telephone between the hours of 0800 and 1600 EST, Monday through Friday, and shall respond to a request for discussion or resolution of technical problems within four hours of notification. 1.4 PERIOD OF PERFORMANCE. The period of performance twelve months from the date of award. Each class shall consist of 15 - 30 students and each class shall take place in no more than 6 weeks. 1.5 PLACE OF PERFORMANCE. The primary place of performance will be contractor's facility. Place of performance may change as necessary to support mission requirements. 2.0 CONTRACTOR FURNISHED ITEMS. Contractor shall provide all supplies and equipment necessary to execute the training requirements for training. 3.0 GOVERNMENT FURNISHED ITEMS. 3.1 Student meals and lodging 3.2 Transportation for students to the contractor's facility 4.0 REQUIREMENTS. This PWS requires the Contractor to provide all EMT training utilizing the following NHTSA References: National EMS Educational Standards, Emergency Medical Technician Instructional Guidelines, EMS Education Agenda for the Future: A Systems Approach, National EMS Core Content, and National EMS Scope of Practical Model. The training will include the following tasks: 4.1 Given a patient in cardiac arrest that is pulseless and apneic, an equivalent to the standard Coast Guard EMT pack, and an Automatic External Defibrillator (AED), ADMINISTER cardiopulmonary resuscitation (CPR) to a patient. CPR is administered correctly IAW current American Heart Association guidelines. 4.2 Upon arrival at scene of emergency, and given an equivalent to the standard Coast Guard EMT pack, a certified high-visibility public safety vest, an N95 respiratory, and a radio, EXECUTE a scene size-up to ensure scene safety and determine EMS needs. Scene safety has been determined and emergency medical needs identified 4.3 Given a patient experiencing an emergency, given an equivalent to the standard Coast Guard EMT pack, a radio, and a patient care report form, CONDUCT a primary assessment of the patient. Patient has been assessed and immediate life threats identified and managed. 4.4 Given a simulated patient who is unconscious or whose airway has been compromised, an equivalent to the standard Coast Guard EMT pack, and a radio, CORRECT a patient's airway. Patient has a patent airway. 4.5 Given a simulated patient who is not breathing adequately, an equivalent to the standard Coast Guard EMT pack, an airway mannequin, a manually triggered ventilator, a humidifier, a Venturi mask, and a radio, CORRECT a patient's breathing. Patient is ventilating satisfactorily and receiving adequate oxygen supply. 4.6 Given a simulated patient with external bleeding and/or signs and symptoms of shock, an equivalent to the standard Coast Guard EMT pack, and a combat application tourniquet (CAT), CONTROL bleeding and shock. Patient's vital signs are stable and external bleeding is controlled. 4.7 Given a simulated patient experiencing a medical emergency and an electronic patient care report, OBTAIN a pertinent patient history in the SAMPLE format. Patient history as it pertains to the present injury/illness is ascertained and documented. 4.8 Given a simulated patient experiencing a medical emergency andfor whom obvious life threats have been identified and managed and from whom a history has been ascertained and documented, an equivalent to the standard Coast Guard EMT pack, a radio, and a patient care report, CONDUCT a secondary assessment of patient to determine detailed prehospital emergency needs. Patient is thoroughly assessed and interventions applied. 4.9 Given a simulated patient with soft tissue wounds and/or burns, an equivalent to the standard coast Guard EMT pack, and a radio, TREAT each wound and/or burn in the correct manner. Patient's wounds and burns are dressed appropriately. 4.10 Given a simulated trauma patient and an equivalent to the standard Coast Guard EMT pack, IMMOBILIZE a patient. Patient is secured to long backboard and ready for transport. 4.11 Given a simulated patient with a musculoskeletal injury, an equivalent to the standard Coast Guard EMT pack, stabilization equipment including traction splints, formable splints, rigid splints, and a radio, STABILIZE a musculoskeletal injury. Patient's musculoskeletal injury is protected from further injury. 4.12 Given a simulated patient whose condition and assessment findings indicate a need for acetaminophen, albuterol, activated charcoal, glucose, diphenhydramine, ibuprofen, aspirin, epinephrine, nitroglycerin, or a prescribed medication already in the patient's possession, ADMINISTER medication to patient. Medication administered correctly IAW the "six rights." 4.13 Given a simulated patient experiencing a childbirth emergency, an equivalent to the standard Coast Guard EMT pack, and a radio or other communication device, ASSIST in the delivery of a newborn. Assistance is provided as directed by Medical Control. 4.14 Given a simulated pediatric patient, an equivalent to the standard Coast Guard EMT pack, and a radio or other communication device, TREAT a pediatric patient correctly. Pediatric patient has stable vital signs and has been provided the appropriate pre-hospital emergency care required. 4.15 Given a simulated patient who needs to be moved and an equivalent to the standard EMT pack, a wheeled stretcher, a stair chair, a sheet, and patient restraints, PERFORM patient moves and lifts to relocate the patient for access or transfer. Patient is moved or lifted effectively without further injury. 4.16 Given a simulated patient ready for transport, an ambulance, a patient care report form, and another EMT, TRANSFER a patient to an equal or higher level of care. Patient is delivered and handed off to the appropriate care facility with all pertinent information. 4.17 Given a simulated mass casualty incident, an equivalent to the standard Coast Guard EMT pack, a public safety vest, and a radio, TRIAGE a mass-casualty incident (MCI) following the Simple Triage and Rapid Treatment (START) system. Patients involved in a MCI have been assessed, prioritized, treated, and transported according to the START system. 4.18 Upon conclusion of the course, the student must take the online NREMT exam for certification at a Pearson View authorized testing facility within 25 miles of the contract facility. The contractor is responsible for transporting the students to and from the Pierson View testing facility.
