NO REIMBURSEMENT WILL BE MADE FOR ANY COSTS ASSOCIATED WITH PROVIDING INFORMATION IN RESPONSE TO THIS NOTICE AND ANY FOLLOW-UP INFORMATION REQUESTS. DESCRIPTION: Small business sources and cooperatives consisting of small business sources (defined as less than 500 employees for NAICS codes 424480 and 311991) are sought that are manufacturers or dealers of fresh of minimally processed fruit and vegetable products. Specifically, small businesses sources are sought to supply/provide FF&V products to school-designated destinations to support schools located in the States of Michigan and/or Florida. Sources are sought for the following FF&V categories: Apples, Blueberries, Grapes, Oranges, Carrots, Lettuce, Broccoli, Strawberries, and other related products. Other related products include those that are minimally processed, such as sliced apples, sliced oranges, chopped lettuce (salad), and other products listed in the contractor's list of items offered for sale. All fruit and vegetable products purchased by USDA-AMS must be 100% verifiable domestic origin, and must be delivered on an as-needed basis to school-designated destinations to support schools. Potential sources are requested to provide a Technical Capability Statement as indicated below for USDA-AMS review. This request is for information only; proposals are not requested nor will they be accepted in response to this notice. No contracts will be awarded solely on the basis of the responses to this notice. Business Information Please provide the following business information for your company and for any teaming or joint venture partners: 1. Company Name and Address 2. Point of Contact (Name, telephone, and e-mail) 3. Size of business pursuant to the North American Industrial Classification System (NAICS) Codes: 424480 and 311991. Please specify your business as either large (greater than 500 employees), small business (500 employees or less), or 8(a) (a contractor eligible under Section 8(a) of the Small Business Act (15 U.S.C. 637(a)). 4. Applicable Dun and Bradstreet Number (DUNS), Tax Identification Number (TIN), CAGE code 5. Have you done business with the Federal Government? 6. If you have done business with the Federal Government, provide a copy of registration on System for Award Management (SAM) Part II. Ability to Manage 1. Briefly describe your company's ability to manage a business, including the management of suppliers/subcontractors. 2. Briefly describe management experience in regards to the specific tasking described, i.e. the delivery of FF&V products to support schools on an as-needed basis. Part III. Technical Capability 1. Briefly describe the capability of your business/facility to provide fresh produce to the destinations identified in Exhibit 2. 2. Describe the capability of you and your suppliers to complete and pass good agricultural practices (GAP) and good handling practices (GHP) audits. USDA/AMS contractors must have a comprehensive food safety system in place which includes verification throughout the supply chain which includes growers, packers, handlers, processors and distribution points. Verification of food safety practices must include the appropriate good agricultural practices/good handling practices/good manufacturing practices (GAP/GHP/GMP) audits conducted either by the UDSA Agricultural Marketing Service, Specialty Crops Inspection Division or by a certification body accredited to perform audits against one of the internationally recognized food safety schemes benchmarked by the Global Food Safety Initiative. (The audit must cover all parts of the operation which are applicable to the FF&V being purchased. For example, an apple purchase would require an audit of the orchards, packing house, and storage facilities.) 3. Briefly describe the distribution system by which you will provide FF&V to the USDA-designated destinations identified in Exhibit 2 in Michigan and/or Florida. 4. Describe how you ensure 100% domestic origin of the FF&V supplied, including the traceability from subcontractors, including growers, processors, and distributers. Provide an example of documentation that specifies the location where the FF&V was grown, processed and shipped. Part IV. Capacity 1. List all current areas of Service or Delivery. 2. Describe your ability to have or obtain a contractual arrangement to provide fresh or minimally processed blueberries, grapes, oranges, carrots, apples, lettuce, broccoli, and strawberries to the school-designated destination identified in Exhibit 2, including documentation supporting the description. 3. Provide a description of comparable work performed within the past 5 years - a brief description of the project, customer name, timeliness of performance, customer satisfaction, and dollar value of the project - provide at least three examples.
