Description

The Department of Veterans Affairs NETWORK Contracting Office 15 is seeking potential SMALL BUSINESS vendors to provide Heart Lung Perfusion Systems with the following items as listed below required for open heart cases and is mandatory to support the open-heart program as Brand Name or Equal that can meet the requirements for the Saint Louis, MO VAMC. The equipment must be compatible in all aspects (form, fit and function) with the existing equipment installed. Location: Department of Veterans Affairs Saint Louis, MO VAMC. If you can meet all of the requirements for this procurement respond to this sources sought by email donald.hannah@va.gov. Responses must be received no later than 15:00 Central Standard Time, on July23, 2013. The following information is required: 1. Data Universal Numbering System (DUNS) & Commercial and Government Entity (CAGE) (This information can be obtained from your Central Contractor Registration). 2. Name of firm w/address, phone and email address of point of contact. 3. State if your company is a certified 8(a), Small Disadvantaged Business, Hub Zone Small Business, or Service-Disabled Veteran Owned Small Business. 4. Provide product brochures that demonstrate that your product can meet the requirements for this purchase. No telephone responses will be accepted. STATEMENT OF WORK This solicitation uses a Brand Name or Equal Description of the product required. This permits prospective contractors to offer products other than those specifically referenced by brand name. All offers must work with existing equipment that has already in use at the station. The Saint Louis VAMC is soliciting quotes to provide Heart Lung Perfusion Systems with the following items as listed below required for open heart cases and is mandatory to support the open-heart program as Brand Name or Equal that can meet the requirements of this RFQ. Vendors that can provide this system Brand Name or equal shall include a copy of the equipment brochure which clearly defines the system or equal. Any information submitted by an offeror for consideration will not be returned and will remain the property of the VA. Description of Requirement is as follows: Provide Heart Lung Perfusion Systems Brand Name or Equal to Saint Louis Veteran Affairs Medical Center, MO. The Heart lung machine will provide life support to patients during cardiothoracic surgery. The unit shall have built in backup battery module with 130 min uninterrupted power with continual time remaining display and must be compatible with CPRS & VISTA. Also, will be used for urgent / emergent usage it is imperative that the unit be fully operational in 3 seconds. To reduce failure rate unit shall have a direct-drive technology and centrifugal pump system with double headed roller pumps with individual controls. The vendor shall provide on-site clinical training as well as web based courses for initial training and refresher training.
