- Information Notice General The VA Medical Center, Syracuse, New York is engaging in a short-list selection of an AE (SDVOSB) for the design of an audiology clinic. Vendors with an SF-330 on file within the past twelve months are being considered, but should review their previously submitted SF330's to ensure all below listed evaluation factors are addressed. Other interested vendors may submit SF-330 information, constructed to address the evaluation factors noted below, to the Contracting Officer at Robert.lee9@va.gov by 2 PM EST, Monday 15 July 2013. A/E Scope of Services The A/E shall provide complete design services for renovations to the existing Audiology Clinic area which has a gross foot print of approx. 2800SqFt. Clinic renovation enlargements will comprise an additional gross foot print of approx. 1800-2000SqFt. The extent of work will be determined from the investigative services, but as a minimum shall include designs for selective demolition of existing walls, ceilings, flooring, electrical/communication/ lighting circuits/systems, as well as fire protection and MEP systems reconfiguration/replacement. The renovations shall also include an asbestos abatement design. Construction design will include floor plans, reflected ceiling plans, equipment layout plans, elevation plans, fire protection plans, MEP schematics, along with wall, ceiling, flooring, door, signage, and finish schedules To include reconfiguration of existing area electrical and communication closets and consolidation into larger closets of all associated circuitry. There will be an increase to the number of Audiometric suites, Audiologist offices, and bathrooms, with enlargement of the Waiting/Reception area, Fabrication area, VGN and Rotary Testing area, and construction of storage closets and a Chief of Services Office. The design will meet all current federal standards as well as all VA design guidelines wherever possible. Services shall include, multi-disciplinary investigative services, initial conceptual drawings, attending required design/review meetings, preparation of contract drawings and specifications, cost estimating services, and construction period services which shall include final punch list compilation and as-built drawings. Deliverables being provided to the VA will include initial conceptual drawings, 35%, 65%, 95%, and 100% drawings and specifications for review and approval, and 100% construction documents provided in multiple sized copies. The following evaluation factors will be used when reviewing submitted SF 330 data and will comprise the basis of information to be used by VAMC Syracuse for A/E evaluation and selection. The evaluation factors are shown below in descending order of importance (highest importance at the top). Firms submitting SF 330 data for consideration must address each of the following factors in the submitted SF 330: 1.Professional qualifications of the team proposed as specifically related to the requirements of this particular project (background of personnel: project manager, key personnel, consultants; project-related professional qualifications; specialized experience (project-related) of team members). 2.Specialized experience of the team proposed for this project (technical competence in the type of work required with specific emphasis on experience related to work at VA Medical Centers). 3.Past Experience/Past Performance: Provide prior prime contractor (SDVOSB) experience on projects awarded within the last three years for hospital work similar in size, scope and complexity to this project. Include the following information for each project listed: -Name, Address, Telephone Number, and Fax Number of a customer point of contact (POC) who would have the most detailed information about the AE's performance. -Project location, award date, original completion date, actual completion date, original contract award amount, final contract amount. Include names, addresses and phone numbers of subcontractors used. 4.Proposed Management Plan and Approach (management plan for the design phase and the, construction phase as well as overall design philosophy). 5.Location (proximity of facilities and personnel to the geographical area of the work; corporate knowledge of local conditions related to the locality of the project) 6.Capacity (to accomplish the work in the required time: assume a 12 August start date with a requirement to provide the 100% design not later than 9 December 2013. 7.Estimating Effectiveness (construction estimate versus construction award amount: provide data/information on the ten most recently completed projects). 8.Project Control (describe techniques used by the AE to control the schedule and costs and identify personnel responsible for these functions). 9.Awards and Recommendations (identify any awards received for design excellence). 10.Insurance and Litigation (identify the type and amounts of liability insurance carried and who the provider is. Identify litigation involvement over the last five years with resulting outcome). This procurement is set-aside for Service Disabled Veteran Owned Small Businesses (SDVOSB). The estimated construction cost for this project is between $600 and $800 thousand. The NAICS code for this procurement is 541310. In accordance with VAAR 805.207, VAMC Syracuse has established a geographic area of 300 miles from Syracuse, NY within which it will consider A/E firms (including joint ventures) for this requirement. This geographic area is determined to be large enough to assure selection of three to five firms highly qualified for this project involved, but not so large as to make the evaluation process unduly burdensome.
