- PRH Dated June 17th, 2013 PRH DATED JUNE 17, 2013 Request For Quote for a Dentist.THIS IS A SUBCONTRACTING OPPORTNITY AT THE LOS ANGELES JOB CORPS CENTER DENTAL SERVICES The Los Angeles Job Corp Center (LAJCC) is requesting bids for a dental service provider wherein a contract will be awarded for the support of 579 LA Job Corps students. LAJCC is a federally funded training and education program for poverty level youth ages 16-24 years. The dental services will be provided in our facility located at 1020 South Olive Street, Los Angeles, CA 90015. The Job Corps Center will provide the dental assistant and dental hygienist. The successful Subcontractor / Dentist must and will be required to: a) work a minimum of 18 hours per week and any additional hours must be approved in advance by the Center Director. b) Subcontractor / Dentist must be licensed in the State of California. c) Must provide Professional Liability Insurance. d) Possess a current Registration Certificate with DEA credentials for dispensing Controlled Substances. e) The dental services shall include general practice: 1) Preventive dental health education. 2) 24 Hour emergency on call coverage including weekends. 3) Oral examination, to include four bite-wings X-rays. 4) Restorative and prophylactic treatment. 5) A diagnosis and treatment plan for each student within two weeks after the completion of a routine dental examination, including a listing of the teeth involved and specific services to be rendered. The base period of performance for the contract shall be thirteen (13) months commencing on October 1, 2013 and continuing until October 31, 2014 with the option to extend the period of performance for up to two years in successive one year increments at the sole discretion of the Los Angeles Job Corps Center.The successful offeror must comply with Federal Acquisition Regulations (FAR); all contract Terms and Conditions and clauses and be compliant under all related federal statutory requirements and regulations. It is the responsibility of the Offeror to obtain the attachments cited below. LAJCC-RFP-42, Attachment 1 identifies LAJCC's specific requirements and expectations to ensure that the dental services that are to be provided by the Center Dentist under this subcontract shall conform, at a minimum, to the dental services and priority system specified in the most current edition of Job Corps Policy and Requirements Handbook (PRH), Chapter 6, Section 6.10.R2 and Technical Assistance Guide (TAG) 6-A which may be obtained from the Dept of Labor website: Job Corps Policy Requirements Handbook online.
