- RFQ-2013-008 N100 Respirators This announcement constitutes a Request for Quotation (RFQ). No formal, written solicitation will be issued. The attachment to this notice entitled "RFQ-2013-008 describes the product requested and provides instructions on how to respond. The United States Senate Office of the Sergeant at Arms (SAA) has a requirement for particulate respirators. The SAA may ask that a sample of for the particulate respirator be provided by Offeror. The SAA intends to award an indefinite delivery indefinite quantity (IDIQ) type contract with fixed prices, to the responsible Offeror with acceptable past performance, whose quotation, conforming to the RFQ as provided herein provides best value to the SAA. The period of performance will be from October 1, 2013 through September 30, 2014. The SAA will issue delivery orders under the contract which shall state the actual quantities per size. All deliveries shall be FOB Destination to the United States Senate, 6330 Columbia Park Road, Landover, MD 20785 and made within ten (10) business days of the issuance of a delivery order. Questions regarding this RFQ must be submitted in writing no later than Noon, July 1, 2013 to acquisitions2012@saa.senate.gov. Responses to all written questions will be posted as an amendment to this Notice. It is the responsibility of all offerors to monitor this site for the posting of the questions posed and the SAA's responses to those questions. The SAA will not respond to questions submitted after the posted deadline or to any telephone requests for information. Oral communications are not acceptable in response to this Notice. SUBMISSION DEADLINE: the completed Quotation form RFQ-2013-08 must be received no later than Noon EST, July 11, 2013. Responses shall be submitted by email to acquisitions2012@saa.senate.gov. The SAA will not accept any faxes or mail deliveries. The subject line of the email message shall be "RFQ-2013-008 Particulate Respirators. The Quotation must contain: 1) Complete RFQ Pricing Table 2013-008; 2) Three past performance references for contracts/purchase orders of similar products within the last two (2) years. Past performance references must provide: a) Name of the Customer/Company; b) Name and phone number of the primary contact with Customer/Company; and c) contract/purchase order value. EVALUATION FACTORS FOR AWARD: The following factors shall be used to evaluate offers: (1) Acceptability of item offered, in terms of whether it meets or exceeds the RFQ requirements; (2) Past performance information for the Offeror; (3) Evaluated price. Responses received without the required information will be considered non-responsive. Access by the SAA to information in any files attached to the quotation is the responsibility of the submitting party. The SAA is not responsible for any failure to access information. The Senate or the SAA will not be responsible for any costs for the preparation of responses to this announcement.
