Sources Sought Number: VA69D-13-I-1567 Notice Type: Sources Sought Notice Synopsis: VISN 12 EOC Mock TJC Survey & TJC Care Education Sessions REQUEST FOR INFORMATION / SOURCES SOUGHT INQUIRY The Department of Veterans Affairs VISN 12, Great Lakes Acquisition Center (GLAC) is seeking information and sources that are capable of providing personnel, equipment, and materials necessary to perform Mock Environment of Care (EOC) Surveys and Care Educational Sessions in accordance with The Joint Commission (TJC) standards for all VISN 12 facilities consisting of Oscar G. Johnson VA Medical Center in Iron Mountain, Michigan; VA Medical Center in Tomah, Wisconsin; William S. Middleton Memorial Veterans Hospital in Madison, Wisconsin; Zablocki VA Medical Center in Milwaukee, Wisconsin; Capt. James A. Lovell Federal Health Care Center in North Chicago, Illinois; Edwards Hines VA Hospital in Hines, Illinois; and Jesse Brown VA Medical Center in Chicago, Illinois. The mock survey process will focus on the performance of functions across the organization as described in the current TJC Hospital Accreditation Standards Manual and other specialty manuals. The purpose of the mock surveys is to identify problems and issues that must be resolved to achieve and maintain compliance with TJC standards. A one (1) half-day VISN-wide educational session presented at each site. The length and focus of this training session may be modified by mutual agreement with the local facility. The one (1) half-day Environment of Care Education session will focus on the new requirements in the Environment of Care Life Safety, and/or Emergency Management Standards for 2013 or current edition, any survey process or procedural updates, current issues that TJC is focusing on, and advice on how to best present VISN 12 programs. Special emphasis will be given to maintain the required documentation, evaluating performance improvement activities, conducting proactive risk assessments, maintaining the 7 EOC Management Plans and required annual evaluations, the annual effectiveness review, tips on preparing for the building tour, and other areas identified by the participants. The target audience for this training will be the Associate Directors, Safety and Health Staff, Facility Managers/Chief Engineers, Biomedical Engineers, Performance Improvement Managers, Risk Managers, Police Chiefs, and others responsible for overseeing the Environment of Care programs. The contractor will prepare the presentation and handouts to be used by the participants. One original copy of the handouts in both hardcopy and electronic form will be sent to the designated VA contact person no later than ten (10) days in advance of the presentation. Mock EOC surveys will be conducted annually at each of the VISN 12 facilities during the first quarter of the fiscal year. The surveys will include the development of a survey agenda, and a review of the seven (7) Management Plans, performance improvement activities, annual evaluations, Statement of Conditions, and other pertinent Environment of Care documents. The contractor will assess the documentation covering fire safety, medical equipment, and utility systems. A review of related Environment of Care documentation from each facility's Community Based Outpatient Clinic (CBOC) is also required. The contractor will provide a surveyor for seventeen (17) days as listed below for the Environment of Care surveys. (Actual dates to be mutually determined by contractor and Contracting Officer's Representative (COR).) MINIMUM QUALIFICATIONS: When responding to this Sources Sought Notice, please indicate your level of expertise on each of the following Minimum Qualifications. "Experience and level of expertise with consulting on TJC standards and/or conducting EOC Mock Surveys. "Experience and level of expertise with consulting on and practical implementation of the TJC Hospital Accreditation Standards Manual. "Past pertinent and relevant experience relative to TJC standards in an analyzing, consulting, or implementing context. "Experience and level of expertise making recommendations and presenting recommendations for implementation related to TJC standards and the Hospital Accreditation Standards Manual. The information submitted shall contain the company's business size status, as well as any sub socio-economic qualifications. This is a request for information and sources only, which may or may not lead to a future solicitation. This is not a request for proposal (RFP). The VA will not pay for any information received resulting from this sources sought notice. Requests for copies of a solicitation shall not be honored or acknowledged.
