Description
This is a request for information only. The intent is to identify potential small business offerors capable of providing non-personal services of a Neurologist to perform Physician Reviewed Assessments of Intra-operative monitoring in the operating room for Orthopedic and Neurosurgery Services at Tripler Army Medical Center (TAMC), Hawaii. The contract term, if awarded, will be a base period for one year plus four (4) one-year options. The base period of performance will commence 22 September 2013 through 21 September 2014. The NAICS code for this proposed procurement is 621111 and the annual size standard is $10 million. The qualifications for this position are as follows: Qualifications: The contract physician must be a licensed, practicing physician in good standing. The contract physician shall be a Doctor of Medicine, be a Board Certified expert in Neurology with qualifications in Neuro-diagnostic medicine granted by the American Board of Psychiatry and Neurology, maintain a Hawaii State Medical License, maintain a current, unrestricted state/federal DEA number and possess no physical or mental restrictions for the safe practice of Neuro-physiology testing. The contract physician must be located on Oahu, Hawaii. Experience: The contract physician shall be formally trained in the interpretation of intra-operative monitoring via a Fellowship in clinical Neuro-physiology and have 12 months of experience performing Physician Reviewed Assessments. Specific Tasks: The contract physician shall: Perform the entire range of Intra-operative Neuro-physiology testing to include: Auditory Evoked Potentials Somatosensory Evoked Potential - Upper Extremity Somatosensory Evoked Potential - Lower Extremity EMG - 1 Limb EMG - 2 Limbs EMG - Cranial Nerves, Unilateral EMG - Cranial Nerves, Bilateral Motor Nerve Conduction - Without F-wave Sensory Nerve Conduction Central Motor Evoked Potential H-Reflex, Amplitude and Latency Study Electroscephalogram (EEG) Muscle Test (one limb, two limbs, head ir neck) Intraoperative Nerve Test EEG Monitoring/Video Record EEG during Surgery Analyze Neurostim Complex Cranial Neurostim Complex All qualified sources are invited to submit capability statements that substantiate their experience and qualifications in performing services within four (4) business days from the date of this notice. Interested firms shall indicate if they have provided similar services to military medical treatment facilities and/or the general public under terms and conditions similar to those stated in this notice. Responses to this notice should include company name, address, and telephone number, point of contact (POC), and brochures which will allow the Government to understand your service offerings and options.
