Description
This is a Sources Sought Notice announcement; market survey for information only, to be used for preliminary planning purposes. No proposals are being requested or accepted with this synopsis. THIS IS NOT A SOLICITATION FOR PROPOSALS AND NO CONTRACT SHALL BE AWARDED FROM THIS NOTICE. Any requests for a solicitation in response to this notice will not be honored. The Southern Regional Contracting Office, Fort Hood, TX, has an upcoming requirement to solicit for and award a non-personal services contract for Analytical Support for Carl R. Darnall Army Medical Center at Fort Hood, TX. It is anticipated that the proposed services will result in a Firm Fixed-Priced contract. Service provider must provide all labor, materials, parts and equipment for the Department of Industrial Hygiene. The type of solicitation issued will be based upon the responses to this special notice. Contact with Government Personnel, other than the Contract Specialist identified at the bottom of this notice, by any potential offeror, or their employees, regarding this requirement are strictly prohibited. The purpose of this synopsis is to gain knowledge of interest, capabilities and qualifications of various members of the Small Business Community to include Section 8(a), Historically Underutilized Business Zones (HUB-Zone), Women-Owned Small Business, and Service-Disabled Veteran-Owned Small Business (SDVOSB), as well as the interest, capabilities and qualifications of various members of the Large Business community, to compete and perform a Firm Fixed Price Contract for these services. The type of solicitation to issue will depend upon the responses to this sources sought synopsis. The Government must ensure there is adequate competition among the potential pool of responsive contractors. The North American Classicization System (NAICS) code for this requirement is 621511 - quote mark Medical Laboratories quote mark. The Small Business Size Standard is $13,500,000.00. For reference purposes, a draft PWS has been attached. Responses to this Sources Sought Notice should demonstrate the firm's ability, capability, and responsibility to deliver the services listed above. In response to this notice, interested parties shall submit a written Statement of Capability limited up to 2 pages and shall include the following information: Offeror's name, addresses, point of contact, phone number, and e-mail address. Offeror's interest in providing a proposal/quote on the solicitation when it is issued. Offeror's capability to meet personnel requirements. Offeror's capability to perform a contract of this magnitude and complexity (include offeror's in-house capability to execute maintenance services for this equipment, comparable work performed within the past 5 years -brief description of the work, customer name, timeliness of performance, customer satisfaction, and dollar value of the work) -provide at least 3 examples. Documentation of technical expertise must be presented in sufficient detail for the Government to determine that your company possesses the necessary functional area expertise and expertise and experience to compete for this acquisition. Offeror's type of small business and Business Size (Whether 8(a), HUB Zone, Women-Owned Small Business, Service Disabled Veteran Owned Small Business, etc.), or Large Business Status, should be documented in the Statement of Capability. Interested parties are responsible for marking information that is proprietary in nature. No reimbursement will be made for any costs associated with providing information in response to this notice or any follow up information requests. The Government is not obligated to, nor will it, pay for or reimburse any costs associated with responding to this sources sought notice. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. The Government will in no way be bound to this information if any solicitation is issued. No subcontracting opportunity is anticipated. A determination by the Government to not compete this proposed contract action based on the responses to this notice is solely within the discretion of the Government. EMAIL IS THE PERFERRED METHOD. PERFORMANCE OF WORK STATEMENT I. SCOPE OF WORK A.1.0. Non-Personal Services: The Contractor shall provide Analytical Support for the Industrial Hygiene Department of the Preventative Medicine at Carl R. Darnall Army Medical Center, Fort Hood, Texas. Contractor shall provide all necessary equipment, Labor, materials and all parts to conduct sample analysis to evaluate potential exposure to a multitude of environmental, chemical, and physical hazards within the worksite, medical facilities, and family housing. The exposures may be to Soldiers, civilian employees, family members, or children. Part of these evaluations may be the exposure to molds and fungi. Performance Requirements: A.1.1. T h e Co n t r a c t o r shall be accredited by the American Industrial Hygiene Association (AIHA) and participate in the AIHA. Environmental Microbiology Laboratory Accreditation Program (EMLAP) testing program. Also provide current accreditation certificates and renewals. A.1.2. The Contractor shall provide analysis as requested and shall provide an immediate E-mail or FAX of analytical results. A.1.3. The Contractor shall provide submit samples in separate batches on a daily basis. A.1.4. The Contractor shall provide a one to three (1-3) day turnaround at a minimum. However, customer may request an analysis be performed within a shorter time frame with appropriate increase to the individual sample pricing. A.1.5. The Contractor shall provide free next day shipment of samples to laboratory for analysis. Contractor Manpower Reporting quote mark ACCOUNTING FOR CONTRACT SERVICES-The Office of the Assistant Secretary of the Army (Manpower & Reserve Affairs) operates and maintains a secure Army data collection site where the contractor will report ALL contractor manpower (including subcontractor manpower) required for performance of this contract. The contractor is required to completely fill in all the information in the format using the following web address quote mark https://cmra.army.mil. quote mark The required information includes: (1) Contracting Office, Contracting Officer, Contracting Officer's Technical Representative; (2) Contract number, including task and delivery order number; (3) Beginning and ending dates covered by reporting period; (4) Contractor name, address, phone number, e-mail address, identity of contractor employee entering data; (5) Estimated direct labor hours (including sub-contractor); (6) Estimated direct labor dollars paid this reporting period (including sub-contractor); (7) Total payments (including sub-contractor); (8) Predominant Federal Service Code (FSC) reflecting services provided by contractor (and separate predominant FSC for each sub-contractor if different); (9) Organizational title associated with the Unit Identification Code (UIC) for the Army Requiring Activity (the Army Requiring Activity is responsible for providing the contractor with its UIC for the purposes of reporting this information); (10) Locations where contractor and sub-contractors perform the work (specified by zip code in the United States and nearest City, Country, when in an overseas location, using standardized nomenclature provided on website); (11) Presence of deployment or contingency contract language, and, (12) Number of contractor and sub-contractor employees deployed in theater this reporting period (by country). (13) As part of its submission, the contractor will also provide the estimated total cost (if any) incurred to comply with this reporting requirement. Reporting period will be the period of performance not to exceed 12 months ending September 30 of each government fiscal year and must be reported by 31 October of each calendar year. The contractor shall notify the Contracting Officer's Representative (COR) by the 5th working day of November whether or not they have completed this report. If the COR is unavailable, the contractor will notify the Contracting Officer. quote mark PERFORMANCE REQUIREMENTS STATEMENT (PRS) The following minimum Performance Requirements Statement applies. The Government may modify inspection methods in accordance with site specific requirements.
