This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice and FAR 13.5, Test Program for Certain Commercial Items. This announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. A Request for Quote under solicitation number VA248-13-Q-1796 is being issued to purchase 17 Fresenius dialysis units. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-64. This is a brand name requirement. This is a total small business set-aside. To be eligible for award, the offeror must be registered with the Dun & Bradstreet and System for Award Management (SAM) prior to award, pursuant to applicable regulations and guidelines.
