This is a sources sought announcement only and is not a request for proposals or quotes. The Government does not intend to make an award on the basis of this Sources Sought Notice or otherwise pay for the information solicited herein. This notice is for the purpose of identifying potential sources and does not commit the Government to make an award and /or issue a solicitation. The Department of Veterans Affairs, Central Alabama Veterans Healthcare System (CAVHCS), Montgomery/Tuskegee, AL is seeking qualified SMALL BUSINESS vendors to provide housing and case management services to the beneficiaries of CAVHCS for a period of one (1) year, with a one (1) year option period. The following information is provided to assist the Department of Veterans Affairs in conducting market research of industry to identify potential contractors to provide the services identified. Negative drug screens will not be a requirement for admission. It is understood that the type of patients to be cared for under this contract will require care and services above the level of room and board. The per diem rate established will be an all inclusive rate. Care will include, but not be limited to: 1.Structured group activities, including physical activities as appropriate. 2.Instruction in and assistance with health and personal hygiene. 3.Monitoring of medications, including locked drawers for storage of medication. 4.Supportive social services, in collaboration with the HCHWHUD-VASH program staff, or other contract resources. 5.Individual case management, including professional counseling on self care skills, adaptive coping skills as appropriate, vocational rehabilitation referrals. 6.Assistance in learning and development of responsible living patterns to achieve a more adaptive level of psychosocial functioning upgraded social skills and improved personal relationships. 7.Support for an alcohol/drug abuse free lifestyle by maintaining a drug and alcohol free environment. 8.Routine and as needed/indicated breathalyzer and urine drug screening. 9.Assistance in learning, testing and internalizing knowledge of the illness/recovery process for homelessness. 10.Employment placement and job training. 11.Housing services to include permanent affordable placement. 12.No pornographic material allowed on the premises. This applies to staff and Veterans. 13.Employees shall not fraternize with Veterans. 14.At least 60% of the veterans discharged from the Contractor shall be discharged into permanent housing. 15.The Contractor shall participate in the local Homeless Coalition and register all residents in HMIS and pay fees associated with HMIS. 16.The Contractor shall maintain washer and dryer facilities at each site for the use of the residents. 17. The Contractor shall provide office space, desk, telephone and fax machine in an area which will ensure privacy, safety, and security. Contractor's facility where services are to be provided shall be within 30 miles of the Central Alabama Veterans Health Care System (CAVHCS) Medical Center East Campus (2400 Hospital Road Tuskegee, AL 36083) or West Campus (345 Perry Hill Road Montgomery, AL 36109). The facility must be located within.50 miles from access to the Public Transit System. Contractor facility shall: 1.Conform to the Life Safety Code, National Fire Protection Association (NFPA) #101 standards 2.Meet all City and State requirements concerning licensing and health codes Contractor shall possess a valid State Business License/Certificate of Occupancy. The applicable NAICS code is 624229 - Other Community Housing Services Firms/Companies with interest and capability responding to these requirements should develop a short narrative including relevant documentation, specific evidence and any other information indicating the capability to successfully fulfill the aforementioned requirements.
