SYNOPSIS: There is a high incidence of sleep disorders within the VA New Jersey Healthcare System (VANJHCS) patient population. Despite the fact that the VANJHCS has a fully functional and highly active sleep laboratory there is an inability to keep up with the demand. We currently have a backlog of 6 weeks and due to the demand for services this demand will likely continue to grow. In order to provide our patients with timely care we seek to develop a contract to perform in-home sleep studies to evaluate sleep apnea, including evaluation of CPAP titration to allow for the treatment of obstructive sleep apnea. This will be in addition to the continued operation of the sleep laboratory, as home sleep studies do not aid in the diagnosis of the remaining litany of sleep disorders. Contractor will send a diagnostic device directly to patients. Contractor will send detailed instructions to the patient with the devices for their sleep studies. They will offer phone support and arrange for the device to be returned to the contractor site via a reliable carrier that will track all shipments. They will then provide a summary of the diagnostic study results. If sleep apnea is diagnosed, a VA physician may request a second study using an auto titrating continuous positive airway pressure device. This test will be performed following the same procedure, to determine an appropriate treatment for the sleep disorder. THE REQUIREMENTS ARE AS FOLLWED: The Contractor shall provide testing and analysis for sleep-disordered breathing to any veteran referred by the VA New Jersey Healthcare System, Department of Veterans Affairs, 385 Tremont Avenue, East Orange, NJ 07018. Initial contact of the patient should include a screening tool approved by the VANJHCS sleep director in order to determine the appropriateness of in-home testing. If the patient's screening determines that an in-home sleep study is not indicated based on comorbidities and/or suspicion of other concomitant sleep disorders, the patient will then be referred back to VANJHCS for in-lab polysomnogram. Contractor shall provide a Sleep Study Report covering three nights of collected sleep data, including respiratory events (apneas and hypopneas, A+H index), snoring intensity, blood oxygen saturation, pulse rate, and respiratory effort. Contractor will analyze the data and provide a comprehensive report to the Non VA Care Coordination program (NVCC). No services should be approved or will be covered without a written authorization from VANJHCS. The vendor must receive an authorization from NVCC prior to sending sleep study equipment to the patient. Contractor shall provide an FDA-approved and clinically validated portable test device for sleep apnea testing at the residence of any veteran. Individually Identifiable Health Information (IIHI) should not be located or stored on portable test devices. In the event IIHI must be located on portable test devices, it must be erased as soon as results have been interpreted. No geographic restriction shall be placed on any veteran to be tested. Contractor physicians interpreting each sleep study must be a board certified/board eligible sleep specialist.
