Endodontic Surgical and Non-Surgical services (Revision 1) This is a PRE-SOLICITATION ANNOUNCEMENT ONLY. The Orlando VA Medical Center/Orlando VA Healthcare System OVAMC (Orlando, FL) intends to make a single small business award for the provision of endodontic services. The endodontic services shall be provided at the contractor place of business located within 15 miles driving distance of the Orlando VA Medical Center 5201 Raymond Street, Orlando, FL 32803 as referenced by Google Maps on the Internet. The contractor shall be capable of providing all necessary care for endodontic services in support of the Orlando VA Medical Center. This includes all necessary care, supplies, facilities, equipment and support services. 1. Services shall be provided Monday-Friday 8:00 a.m.to 5:00 p.m. excluding government holidays. 2. Contractor shall be able to handle expected volume based on history of past analysis on volume of procedures. 3. Perform all non-surgical endodontic treatment. 4. Perform all surgical endodontic treatment. 5. Prepare and teach planned endodontic topics both didactic and clinical for Advanced Education in General Dentistry (AEGD). 6. Routine dental exams shall be scheduled within (7) calendar days, treatment shall be completed within (30) days of initial patient contact or per case by case report. 7. Urgent exams shall be scheduled within (1) business day. 8. All requests shall be completed upon delivery of digital x-rays within 30 days of appointment date. 9. On-call 24/7 provisions for dental emergencies. 10. Providers shall be privileged and credentialed by the contractor; providers shall have a current valid Florida Dental license, graduated from an American Dental Association (ADA) accredited school obtaining a Doctor in Dental Surgery (DDS) OR Doctor in Medical Dentistry (DMD). Providers shall be an endodontic specialist that has completed the minimum post graduate training required by the ADA in an accredited program and completed endodontic training through a 2 or 3 year residency. Providers shall be able to provide proof of completion and qualifications. The contract type is Indefinite Delivery Indefinite Quantity (IDIQ) price contract with firm-fixed unit prices. The contract shall consist of a 12 month base period and four 12 month option periods for renewal. The solicitation will be made available on or about June 10, 2013 and will be solely distributed through the General Services Administration's Federal Business Opportunities website at www.fedbizopps.gov. Hard copies of the solicitation will not be available. This site provides downloading instructions. All future information regarding this acquisition, including solicitation amendments, will also be distributed solely through this site. Interested parties are responsible for monitoring this site to ensure that they have the most up-to-date information about the acquisition. The anticipated date for receipt of proposals is July 01, 2013. All responsible sources may submit an offer which will be considered.
