VA248-13-R-0208 Questions and Answers 1 Q1.On the Unit Price fill-in form (pages 5 through 17) the following items appear: a.# 3 Antibody Identification/Capitated b.# 7 Antiglobulin-Directed/Capitated c.# 17 Cell Elution ID/Capitated d.#20 Crossmatch Electronic Capitated e.# 21 Crossmatch Serological Capitated f.#58 Type and Screes Capitated All of these items fall under Compatibility Services. With a Capitated Compatibility Agreement, the first 3 items are included at No Charge, while the last 3 items are all considered as "Procedures." The capitated agreement provides for a range of "Procedures" for a certain monthly flat rate. I can either leave these 6 items blank, or mark them as "Included in Capitated Compatibility Agreement" since they will be covered in the later Solicitation. A1. Solicitation Amendment A00002 is posted which revises the Supplies/Services list. The items listed above are excluded from the revised list. Q2. Instead of filling-in the Unit Price forms (pages 5 through 17), can we submit a two-page spreadsheet with the exact same information? Or, would you prefer to have us fill in the forms and also submit the spreadsheet as supplemental information? A.2 Either is acceptable as long as it is the items/services that are requested in the solicitation. Q3. Do you need copies of any of our licenses or registrations - for example our FDA Registration? A3. Please submit copies of registrations, certifications or accreditations from: The Food and Drug Administration, The American Association of Blood Banks (AABB), The College of American Pathologists (CAP), or the Joint Commission.
